What are the responsibilities and job description for the Registered Nurse/LPN position at Auburn Post Acute?

Duties:
- Perform routine patient assessments and provide appropriate nursing care
- Administer medications and treatments as prescribed by physicians
- Monitor and document patient vital signs and symptoms
- Assist with specimen collection and processing
- Provide infant care, including feeding, diapering, and monitoring developmental milestones
- Assist with aliment administration and dermal filler procedures
- Provide health coaching to patients on various topics such as nutrition, exercise, and medication management
- Collaborate with interdisciplinary healthcare team members to develop and implement individualized care plans
- Coordinate discharge planning and ensure smooth transitions for patients leaving the healthcare facility
- Provide specialized care for patients with dementia, including cognitive stimulation activities and behavior management techniques
- Utilize PointClickCare or similar electronic health record system for documentation and communication purposes
Experience:
- Must have a valid RN/LPN license in WA state
- Minimum of 1 years of nursing experience in a clinical setting
- Experience in patient education is a plus
- Familiarity with discharge planning processes and resources available in the community
- Experience in providing specialized care for patients with dementia, including knowledge of behavioral interventions and therapeutic activities

Open Schedules:
Full-Time Day Shift: 6am-6pm, 2 on 3 off, 2 on 2 off, 3 on 2 off rotating schedule
Full-Time Treatment Nurse: 2pm-10pm, 5 days per week
Part-Time Treatment Nurse: 2pm-10pm, 3 days per week
Job Types: Part-time, Full-time
Pay: $38.00 - $45.00 per hour
Expected hours: No less than 24 per week
