What are the responsibilities and job description for the Care Manager position at AUNT MARTHAS HEALTH AND WELLNESS INC?
SUMMARY
The Care Manager is responsible for providing the supervision and support to the Associate Care Manager, Care Coordinators and the education staff. The purpose of this position is to ensure the provision, integration and coordination of all needed services to the youth in the program. Services are intended to enhance the youths’ level of functioning with the overall goal of coordinating their treatment needs and facilitating movement towards permanent placement.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.
Ensure compliance and adherence to all Department of Children and Family Services (DCFS), Licensing, Agency and Joint Commission standards, policies and procedures.
Ensure appropriate youth services are implemented and coordinated on an on-going basis. Services include physical health, medication management, legal, educational, social and psychiatric.
Participates in routine staffings with DCFS administration for all prospective admissions to the ICC and ICC Step down program.
Ensure the timely completion and submission/entry of varying youth related documentation by assigned staff.
Ensure staff provide and document therapeutic and skill building interventions, as dictated by the youths’ treatment/care plans, in individual and group based settings.
Ensure all programmatic reports are completed and submitted within the prescribed timelines.
Attend and participate in Discharge / Clinical Intervention Placement Preservation (DCIPP and CIPP) staffings, Care Coordination meetings, staff meetings, and any other case related meetings as designated by the Senior Vice President.
Provide transportation to youth as needed in agency or personal vehicle.
Ensure all youth records are maintained within agency and funder guidelines.
Evaluate program performance and participate in short and long-term planning.
Evaluate, identify and respond to, in a timely manner, the training needs of the program. Provide all forms of training, consultation, evaluation, direction and role modeling to staff in order to enhance and develop programming.
Participate in internal/external committees as needed.
Ensure high quality communication (written and oral) is presented by the staff and program.
Fully and professionally respond to and implement all directives and advice offered by all supervisory and training personnel as well as professionally offered feedback from coworkers.
Notify leadership of all potential high risk situations in a timely manner.
Assist with special projects as designated by the leadership team.
Participate in clinical supervision and program meetings as required.
Participate in agency and DCFS initiatives as indicated.
Ensures compliance with agency personnel file.
Complete all Core Curriculum training(s) as required for the position
Perform all other duties as assigned
WORK HOURS
Willing to work varying hours based on the need of the program. Hours may include evenings and weekends.
Report to work site on time and fully prepared to engage in all job responsibilities.
If qualified, willing to cover job responsibilities of co-workers or subordinates in the event of an emergency or shortage as assigned by Supervisor.
SUPERVISORY RESPONSIBILITIES
Directly supervises a team consisting of Care Coordinators, Associate/Educational Specialist and Associate Care Manager. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; addressing complaints and resolving problems.
QUALIFICATIONS
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.
EDUCATION and EXPERIENCE
Master’s Degree from an accredited school of social work or in a human service field, two years full time experience working in a social work setting and two years supervisory experience is required. LPHA (License Practitioner of the Healing Arts) designation strongly preferred.
OTHER QUALIFICATIONS
IM CANS Certification – if does not possess at time of hire must have completed within two weeks of hire.

Current auto insurance
Clear driving record
Good Working knowledge of Microsoft applications
The ability to critically think, work well under stress and work well in crisis-oriented situations.
The ability to maintain a high level of confidentiality.
