JOB DESCRIPTION SUMMARY
The Self Determination Program Coordinator is responsible for processing client referrals, service provider/vendor enrollment packets, client authorizations and all other forms associated with the Self Determination Program at Aveanna. The SDP Program Coordinator facilitates service provision for a client/family from the initial referral through to case activation, time entry, payroll and invoice payment. This position is also responsible for editing and monitoring time entry every pay cycle as it relates to program regulations and policies. An SDP Program Coordinator is the primary point of contact for Aveanna clients and contributes to growing and maintaining relationships with clients, service providers and referral sources through exemplary service.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES:
1. Adhere to Aveanna Core Values
2. Answer and transfer calls on multi-line phone system
3. Process referrals, service provider/vendor enrollment packets, and authorizations
4. Process, edit and monitor time entry every payroll cycle
5. Ensure time entry deadline is met for assigned regional center
6. Follow up with regional centers, clients, families, and service/care providers as needed to execute duties
7. Resolve payroll issues or concerns with clients, families and regional center staff
8. Run monthly CPR/FA expiration reports for service/care providers
9. Follow up with inactive families referred
10. Assist with general office correspondence and answer all direct correspondence
11. Assists clients in the office with questions, inquiries, and paperwork
12. Prioritize tasks from supervisors and managers
13. Effectively communicate with all levels within the organization
14. Create/send monthly budget reports using a specified template
15. Read and interpret client spending plan
16. Make purchase and AP payment requests
17. Send and follow-up on background checks sent to the Department of Developmental Services (DDS)
18. Consult and assist clients and regional center staff as necessary for spending plan creation and
certification (i.e. taxes and OT)
19. Occasionally visit Regional Center for meetings and events
20. Perform other duties as assigned or needed

POSITION QUALIFICATIONS:
1. Care about the developmentally disabled population
2. Some college or bachelor’s degree is strongly preferred
3. Minimum of one year of payroll and/or customer service experience within the last four years
4. Excellent interpersonal skills; self-driven, motivated and results oriented
5. Strong analytical and judgment skills
6. Intermediate computer skills using Microsoft Windows, Word and Excel
7. Ability to successfully handle multiple tasks simultaneously; strong organizational skills
8. Thrive in a fast-paced environment
