We are seeking a compassionate and dedicated Individuals to join our team as an SMDI Case Manager.
Severe and Disabling Mental Illness (SDMI) Waiver Case Management is a program offered through AWARE for people 18 years and older who require long-term community support due to mental health symptoms.

Responsibilities:
- Provide case management services to adults with severe and disabling mental illnesses as defined by the State of Montana.

Requirements
- Ability to, under AWARE’s Unconditional Care Philosophy, develop and implement individual treatment plans with the knowledge of behavioral, clinical, and mental health interventions, skill building techniques, and understanding of the emotional and behavioral needs of assigned clients.
- Must have the interpersonal oral and written communications skills to work as a team member, to make observations and to report them accurately, to learn behavioral, clinical, and mental health care methods, and to follow oral and written instructions.
- A BS/BA Degree in a Human Services Field.
- Minimum of one (1) year of direct work experience.
