Primary Responsibilities/Essential Functions:
- Works in conjunction with the transportation coordinator (if applicable) to schedule medical appointments for residents.
- Transports residents to medical appointments.
- Completes all follow-up paperwork and record documentation after medical appointments.
- Completes coordination of care notes daily.
- Completes the contact sheet in a timely manner.
- Performs medication evaluations for the DMA’s along with the Regional Operations Managers and Regional Nurses.
- Communicates physician’s request to the pharmacy.
- Obtains resident’s medication consents for psychotropic medications for external providers.
- Ensures resident health care appraisals are completed timely.
- Reports monthly infection control information to Medical Department.
- Responsible for all medication tracking.
- Ensures all medical filing is done within 24 hours.
- Assists medical staff with required training courses for residential care staff and assists with DMA in-services.
- Verifies resident insurance prior to or at intake.
- Completes the new resident medication plan and pharmacy contact information.
- Attends all psychiatric evaluations and medical reviews as requested.
- Completes a monthly inventory on all medical supplies.
- Participates with intake process, transfer process and discharge process from start to finish.
- Responsible for managing site/house/home when the Home Manager is absent.
- Completes and submits requested reports to BSLS as requested.
- Performs other duties as assigned or requested.

Education and Requirements:
- High School diploma or GED required.
- 1-2 years previous experience in a medical office setting or coursework in the medical field preferred.
- Ability to complete required training in connection with The Joint Commission, CHMs and other government bodies.
- Must possess a valid Driver’s License.
- Strong computer skills, preferably using Microsoft Office.
- Proficient in speaking, reading and writing the English language required
Physical Job Requirements:
- Potential exposure to infectious disease.
- Potential for physical aggression from residents exists.
- Ability to lift files, open drawers and bend or stand as necessary.
- Ability to speak, read and write the English language. Work is usually performed in a home-like setting and in community facilities with potential of exposure to infectious disease. Universal precautions are required to minimize the risk of infections. Work sites may include locations that are no barrier-free. Duties are preformed in an environment where the potential for physical aggression from residents
