GENERAL SUMMARY:

The Nurse Navigator will assess the patient’s individual needs and develop a plan of care to meet those needs. This role includes providing education and support to patients, families, staff and caregivers. The goals of this position include improving patient care and flow, eliminating barriers, and improving the overall patient experience by connecting the patient and family with appropriate resources and services. This position is designed to facilitate an effective process of the new care model; supporting quality patient care, safety and financial components; promoting integration of a seamless care model; coordinating patient discharge planning for post-acute services, length of stay (LOS) management and utilization management; collaborating to include coordination as evidenced by metrics, optimizing performance and adoption of best practice. Serves as a leader and resource for the health care team. Integrates new knowledge and evidence-based research into clinical practice. This position reports to the Clinical Nurse Manager and/or the Director of Nursing.

ESSENTIAL DUTIES:

1. Determine the appropriate appointment scheduling, by assessing patient pain, severity of injury, and schedule with appropriate physician group. 2. Responsible for coordinating the care of patients in assigned service line. 3. Knowledge and ability to ask appropriate clinical questions, and illicit patient information to determine appropriate action(s). 4. Priorities include improving patient care and flow, eliminating barriers, and improving the overall patient experience by connecting the patient and family with appropriate resources and services. 5. Access to multiple network clinical information systems to gather and verify patient demographic and clinical information. 6. Collaborate with all care providers to assist in the provision of quality, cost effective patient care. 7. Provide individualized support, education and information, helping to make care seamless, continuous and

comprehensive. 8. Assist in identifying and documenting issues related to transitions of care and will round on patients as appropriate while monitoring their healthcare experience to ensure all expectations are being met. 9. Manage continuum of care from client facilities, giving feedback to and from Beaumont Health. 10. Serve as a liaison between the patient and their family and the various hospitals and physician office professionals with whom they interact. 11. Maintain a full understanding of the services provided by Beaumont Health hospitals, ordering physicians, referral centers and other organizations in the community. 12. Responsible for patients within the inpatient and outpatient setting.

Supports and participates in a collaborative team-oriented environment – cooperates and works together with all co-workers, plans and completes job duties, uses appropriate communications in sensitive and emotional situations and follows up as appropriate regarding reported complaints, problems and concerns.

6. Supports, cooperates with and demonstrates safe work practices and attitudes, follows safety rules – including universal precautions - reports and prevents/corrects unsafe conditions and behaviors, and participates in organizational and departmental safety programs.

7. Completes all required compliance standards that may be department specific and/or identified by the organization.

8. Maintains current licensure, registration and/or certification, as applicable, at all times.

STANDARD QUALIFICATIONS:

A. Education / Training:

  • Bachelor of Science in Nursing required.

B. Work Experience:

  • Minimum 3 years of nursing experience required.

C. Certification, Licensure, Registration:

  • RN License in State of Michigan required. Certification preferred.

D. Other Qualifications:

  • None.

WORKING CONDITIONS:

A. Physical Effort:

  • Strength: Position requires ability to frequently push/pull up to 30 lb. objects with assist of others, occasional requires ability to lift objects more than 50 lbs. with others, constantly lift objects up to 20 lbs. And frequently lift up to 40 lbs.
  • Manual Dexterity: Position requires incumbent to constantly perform simple motor skills such as standing, walking and simple manipulative skills such as writing. Position requires incumbent to frequently perform moderately difficult manipulative skills such as positioning patients.
  • Coordination: Position requires incumbent to constantly perform gross body coordination such as walking. Frequently perform tasks which require hand-eye coordination such as assisting patients to eat, collections of specimens for transport to lab and tasks which require arm-hand steadiness such as taking patients temps and pulses.
  • Mobility: Position requires incumbent to constantly walk and stand; occasionally sit for prolonged periods, frequently bend such as when caring for a patient in bed.
  • Visual Discrimination: Position requires incumbent to constantly see objects far away as in driving and see close objects such as reading thermometers.
  • Hearing: Position requires incumbent to constantly be able to hear normal sound with some background noise as in answering phone/intercom. Occasionally be able to distinguish sound as in equipment alarms.

B. Work Environment:

Employee may be:

  • frequently exposed to electro-magnetic radiations as in VDTs.
  • occasionally exposed to hazardous material other than blood, body tissues, or fluids, dust, toxins, cytotoxins, chemical hazardous materials, bodily injuries, loud and/or unpleasant noises, high humidity, electrical hazards, or poisonous substances.
  • infrequently exposed to blood, body tissue, or fluids.
  • seldom exposed to fluctuations in temperature, grease, oil, and radiation.
  • Potential risk of an exposure to hazardous medications or waste materials of hazardous medications.
