POSITION: ACT TeamCase Manager (Full Time Regular)
SHIFT: Monday – Friday @ 8am to 5pm
LOCATION: BHS ICS – Gardena, CA
DUTIES: Assertive Community Treatment (ACT) Case Manager with the CCBHC will provide goal-directed, client-centered, medically necessary services and support to adults with severe and persistent mental illness exhibiting an increase in symptoms decreasing their ability to manage safely in the community. Specialist will be available to provide one on one service in the home and/or community to the client as well as assisting with coordination of care through treatment planning and referrals. The specialist will be working with shortened timelines for task completion to ensure client’s needs are met quickly with the goal of stabilization of acute symptoms. Computer documentation of all services in a timely manner is required.
- Case management using identified and accepted methods to remediate symptoms of mental illness and/or improve emotional and functional well-being while meeting the expectations of the CCBHC ACT model.
- Conduct needs assessments to support target case management services
- Participate in Treatment Plan development with support of client and family participation on cases where needed
- Attend all ACT team meetings in order to facilitate an integrated, multidisciplinary team approach to service.
- Daily Team Meeting – to organize work for each day and to review recent contact with each client.
- Treatment Planning Meeting – weekly or bi-weekly to develop and review treatment plans/decisions.
- Collaborate in review, updates and progress on goals with Treatment Plan coordinator or other members of the treatment team
- Meet with client to complete strengths assessment, discuss treatment goals and available services
- Complete progress notes in accordance with agency guidelines which meet requirements for medical necessity, goal-directed treatment and client response to treatment within prescribed timeliness
- Assist with capturing and entering outcome measures as directed
QUALIFICATIONS: Following are the qualifications are preferred to perform the essential functions of this position. Qualifications may be subject to modification based on the Americans with Disabilities Act. Bilingual abilities may be required within certain BHS programs.
- Bilingual Spanish preferred.
- Bachelor’s degree or be equivalently qualified by work experience or a combination of work experience in the human services field and education, with one year of experience substituting for one year of education. If no degree is present, applicant may substitute each one year of experience for one year of education.
- Skills and competence to establish supportive, trusting relationships with persons experiencing severe mental illnesses and co-occurring substance use disorder. Respect for individual rights and personal preferences in treatment are essential.
- Training and/or lived experience in substance abuse programs and treatment, relapse prevention, harm reduction, motivational interviewing and 12-Step programs.
- Must have valid California Driver’s license, reliable vehicle and adequate insurance.
- Recovering individuals must have a minimum two years clean, sober and abstinent to be considered for employment.
