Location: Titletown, 1970 S Ridge Rd, Green Bay / Oconto, Marinette and MI UP.
Shift Hours:
Full-Time 1.00 FTE (40 hours/week), Mon-Fri, 8-5
Job Description:
Serve as a care navigator for patients and families receiving hip or knee replacements. Coordinate patient care from the decision of joint replacement surgery through their surgical journey and recovery process to ensure they get a safe and exceptional experience. Provide education to patients before and after surgery to give them the necessary education they deserve. Help patients navigate through their surgical journey so that their primary focus can be on their recovery. Be the patient’s primary contact for all their total joint needs pre- and post-surgery and assist with navigating additional resources as needed. Communicate with patients post operatively to evaluate pain management and provide any additional education. Decrease the number of unneeded touches to the patient.
Qualifications:
WI and/or MI RN License and Associate’s Degree in Nursing (ADN) required. The RN must complete their BSN within five years of the hire date. Knowledgeable in computer and keyboarding skills, excellent verbal and written communication, team player, ability to function independently and autonomously, able to multi-task, organized and sensitive to patient needs are also required. Encouraged to seek life-long learning opportunities, such as obtaining Orthopedic Nursing Certification (ONC).
Two years of orthopedic experience in a clinical setting is preferred but not required.
Additional information:
Position will require travel to Oconto/Marinette WI as well as the Upper Peninsula of Michigan. 20-60% of work will be in the Green Bay area. Amount of travel can be flexible based on preference of successful candidate. Compensation for travel time is included.
WI or MI RN license required upon hire. Both license required within 6 months after hire with Bellin reimbursing for one of the two licensures.
Responsible and accountable for providing quality, safe, professional nursing care to patients, their families and caregivers. Adhere to the WI Nurse Practice Act, Administrative Code of the WI Board of Nursing, and/or appropriate nursing standards governing nursing in the state of practice, the ANA Nursing Scope and Standards of Practice, the ANA Code of Ethics for Nurses and Bellin Health Policies and Procedures. The responsibility includes providing a safe environment for patients, families, staff and visitors; working collaboratively with the multi-disciplinary health care team to provide the best team-based care for the patient; and ensuring they get the necessary education and information in the appropriate format and timing.
