Responsibilities and Duties:
1. Conduct comprehensive assessments of elderly patients, including medical history, physical examination, and diagnostic tests.

3. Provide primary and preventive care to geriatric patients, including managing chronic diseases, prescribing medications, and ordering necessary laboratory tests.
4. Educate patients and their families about disease management, treatment options, and healthy lifestyle choices.
5. Assist in coordinating care transitions for geriatric patients between different healthcare settings, ensuring continuity of care and patient safety.
6. Conduct regular health screenings and preventive services tailored to the unique needs of older adults, including immunizations, osteoporosis screenings, and cancer screenings.
7. Collaborate with interdisciplinary teams to develop and implement programs focused on geriatric patient care and health promotion.
8. Provide counseling and support to geriatric patients and their families regarding end-of-life care planning, advanced directives, and palliative care options.
9. Participate in research activities and quality improvement initiatives within the field of geriatric medicine.
10. Stay up-to-date with the latest advancements in geriatric medicine through continuous education and professional development.
Qualifications and Skills:
1. Master's degree in Nursing with specialization as a Family or Adult-Gerontology Nurse Practitioner.
2. Current and unrestricted Nurse Practitioner license in the state of practice.
3. Certification as an Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP-BC) or Family Nurse Practitioner (FNP-BC) by a recognized credentialing body.
4. Minimum of 2 years of clinical experience providing primary care to geriatric patients preferred.
5. Strong knowledge of geriatric healthcare management, including age-related conditions, medication management, and end-of-life care.
6. Excellent communication and interpersonal skills, with the ability to effectively collaborate with a diverse healthcare team and educate patients and their families.
7. Proficient in using electronic medical records (EMR) and other healthcare software systems.
8. Ability to work independently, make sound clinical judgments, and manage time efficiently.
9. Compassionate, empathetic, and patient-centered approach to care.
10. Strong critical thinking, problem-solving, and decision-making skills.
