Job Summary:
We are seeking a highly skilled and compassionate Nurse Practitioner specializing in Geriatric Medicine to join our healthcare team. Responsibilities and Duties:
1. Conduct comprehensive geriatric assessments, including medical history evaluation, physical examinations, and cognitive function assessments.
2. Develop and implement individualized care plans for elderly patients, considering their medical conditions, functional abilities, and psychosocial factors.
3. Prescribe and manage medications and treatments, monitoring their effectiveness and potential side effects.
4. Collaborate with interdisciplinary teams to address the complex healthcare needs of geriatric patients, including coordinating with specialists, social workers, and therapists.
5. Educate patients and their families on disease prevention, symptom management, and coping strategies for age-related health issues.
6. Provide guidance and support to caregivers and assist in navigating the healthcare system and available community resources.
7. Perform minor procedures and interventions, such as wound care, joint injections, and skin biopsies.
8. Keep accurate and up-to-date patient records, including medical history, progress notes, medication orders, and diagnostic reports.
9. Stay informed about the latest advancements and best practices in geriatric medicine through continuous professional development and active participation in conferences and workshops.
10. Adhere to ethical and professional standards of care, ensuring patient confidentiality and respecting patients' rights.
Qualifications and Skills:
1. Valid Nurse Practitioner license in the state of practice.
2. Master's degree in Nursing specializing in Geriatric Medicine.
3. Certification as a Geriatric Nurse Practitioner (GNP) or Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP).
4. Proven experience working with geriatric patients, preferably in specialized geriatric clinics or nursing homes.
5. Strong clinical skills in conducting geriatric assessments, diagnosing and managing age-related diseases, and promoting healthy aging.
6. Excellent communication and interpersonal skills to effectively interact with patients, families, and other healthcare team members.
7. Ability to work independently as well as collaboratively in a team-based healthcare environment.
8. Proficient in electronic medical record (EMR) systems and computer literacy.
9. Knowledge of Medicare guidelines and reimbursement processes for geriatric healthcare services.
10. Ability to adapt to changing healthcare regulations and standards.
