Position Summary:
The Social Services Director at Bethel Lutheran Home will oversee and coordinate social services programs to enhance the quality of life and well-being of our residents. This role involves providing comprehensive support to residents and their families, addressing psychosocial needs, facilitating discharge planning, and ensuring compliance with regulatory standards. The ideal candidate will possess strong leadership abilities, excellent communication skills, and a deep commitment to resident-centered care.
Key Responsibilities:
- Resident Support and Advocacy:
- Conduct psychosocial assessments to identify residents' needs, preferences, and goals.
- Develop, implement, and regularly update individualized care plans in collaboration with the healthcare team.
- Provide counseling and support to residents and their families, addressing emotional, social, and adjustment issues.
- Advocate for residents' rights and ensure their needs are met in a compassionate and timely manner.
- Discharge Planning:
- Coordinate discharge planning activities to ensure a smooth and safe transition for residents returning home or to another care setting.
- Collaborate with residents, families, healthcare providers, and community resources to develop comprehensive discharge plans.
- Arrange for necessary services, equipment, and follow-up care to support residents post-discharge.
- Community and Family Engagement:
- Serve as a liaison between the facility, residents, families, and community resources.
- Facilitate family meetings and care conferences to discuss residents' progress and care plans.
- Provide education and resources to residents and families on available community services, benefits, and support networks.
- Regulatory Compliance and Documentation:
- Ensure all social services practices comply with state and federal regulations, as well as facility policies.
- Maintain accurate and up-to-date documentation of social services assessments, interventions, and outcomes in residents' medical records.
- Participate in regulatory surveys and inspections, preparing necessary documentation and reports.
- Team Collaboration and Leadership:
- Lead and supervise the social services team, providing guidance, support, and professional development opportunities.
- Collaborate with interdisciplinary team members to promote holistic and integrated care for residents.
- Participate in staff meetings, in-service training, and quality improvement initiatives.
- Program Development and Implementation:
- Develop and implement social services programs and activities that promote residents' psychosocial well-being and quality of life.
- Evaluate the effectiveness of social services programs and make adjustments as needed to meet residents' needs and preferences.
- Other Duties as Assigned.
Qualifications:
- Education:
- Bachelor's degree in Social Work, Psychology, or a related field preferred.
- Associates Degree Required
- Licensure/Certification:
- Current Social Work license in the state of CA preferred.
- Certification as a Certified Social Work Case Manager (C-SWCM) or Certified Advanced Social Work Case Manager (C-ASWCM) preferred.
- Experience:
- Minimum of 2-3 years of experience in social work, preferably in a healthcare or long-term care setting.
- Previous experience in a leadership or supervisory role preferred.
- Knowledge of geriatric care, psychosocial issues, and discharge planning.
- Skills and Abilities:
- Strong leadership and team management skills.
- Excellent communication, interpersonal, and organizational skills.
- Ability to handle sensitive and confidential information with discretion.
- Proficiency in electronic health records (EHR) systems and documentation.
Job Type: Full-time
Pay: $24.00 - $27.00 per hour
Expected hours: No less than 40 per week
