POSITION SUMMARY: Support the goals and challenges of BHcare by providing community support services which focus on developing individualized goals with consumers around a valued role that will increase their success and overall quality of life. This will be accomplished through skill building and motivational support for pursuing goals related to employment, education, community involvement and use of natural supports. Assist the Program Manager with the daily operation of the program.

ACCOUNTABLE AND RESPONSIBLE FOR THE FOLLOWING RESULTS:

  • Assist the Program Manager in the daily operations of the program.
  • Serve as primary contact for program needs, in the absence of Program Manager unless it is for an extended period of time and then the Director will assume responsibility.
  • Assist in the orientation and training for program staff.
  • May review biweekly timesheets for program staff in preparation for Program Managers approval.
  • Assist the Program Manager in on-going development of program policies and procedures and Quality Assurance and Improvement activities as related to program needs and requirements. Provide input to Program Manager for employee performance evaluations.
  • Provide individual and/or group sessions while meeting caseload and productivity requirements.
  • Conduct a functional assessment upon admission and update it every two years (i.e. CASIG).
  • Develop a person-centered recovery plan with stage-wise treatment goals and objectives that are measurable and attainable and ensure it is reviewed at least every 3 months. Observe record and evaluate consumers progress towards goals.
  • Document stage-wise interventions per the psychiatric rehabilitation skill building model. Provide outreach & crisis intervention to community support service consumers as needed.
  • Provide individual and group sessions in the community focused on the appropriate stagewise intervention within the psychiatric rehabilitation skill building model developed by the agency (e.g. assisting with ADLs, teaching recovery skills, develop self-advocacy skills and provide health and wellness education). Provide limited transportation as needed while clients learn individual transportation skills.
  • Provide education, support and consultation to family members, significant others and community members to meet the rehabilitation goal(s) set by the consumer.
  • Develop individualized and generalized community supports.


MINIMUM QUALIFICATIONS

  • Bachelors degree required.
  • Minimum of 3 years experience in a community mental health environment preferred.
  • Knowledge of Psychiatric Rehabilitation is desirable.
  • Must have and maintain a valid CT drivers license if relocated to CT as a state resident more than 30 days ago. Must maintain an acceptable driving record and be insurable at a reasonable rate under the Organizations auto insurance plan.
  • Must be proficient with PCs and Microsoft Office Word & Outlook, and if applicable ability to navigate through electronic health record.
