Description
Position Summary: Support the goals and challenges of BHcare by overseeing the daily operations of the YAS Residential Program which focuses on developing individualized goals with consumers around a valued role that will increase their success and overall quality of life. This will be accomplished through intensive skill building and motivational support for pursuing goals related to employment, education, community involvement and use of natural supports.
Accountable and responsible for the following results:
Supervisory:
- Oversees the functions and daily operations of the YAS Residential Program
- Directly supervise all residential staff
- Ensures that the staff are integrating the ARC model and provide intensive, trauma-informed services; the psychiatric rehabilitation model, and the principles of IDDT into the program services
- Ensures that the LIST assessment and the UM Tool is completed within 30 days of a client’s admission as well as a planned discharge. In addition, it needs to be updated every 3 months in collaboration with YAS OOC liaison
- Interviews new hires with DRS; YAS PM trains new staff.
- Attend all Point Person meetings and any other identified YAS Initiatives. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Provides On-Call back up as part of management rotation. Quality Management:
- In collaboration with the Director develop and implement quality measures noted in the departmental work plan to monitor the effectiveness of staff
- Coordinates with DMHAS Liaison regarding program referrals and client progress.
- Supply data in a timely fashion to the Director to fulfill reporting requirements
- Monitor and audit the program records at least monthly to assure compliance with standards and provide direction/ consultation to staff to ensure work is being performed according to established policies
- Monitor contacts/intensity of client care to ensure programmatic standards are met.
- Meet YAS fidelity standards
Direct Care:(when applicable)
- Conduct a functional assessment (i.e. LIST) and complete the UM tool within 30 days of admission and every three months thereafter.
- Develop a person centered recovery plan with stage-wise treatment goals/objectives that are measurable/ attainable and reflect one of the core domains on the LIST
- Document stage-wise interventions according to the psychiatric rehabilitation skill building model that reflect one of the core domains noted on the LIST
- Observe, record and evaluate consumers progress towards goals
- Provide crisis intervention to residents as needed
- Provide individual/group sessions in the residence/community focused on the ARC model and appropriate stage-wise intervention within the psychiatric rehabilitation skill building model developed by the agency (e.g. assisting with ADL’s, teaching recovery skills, develop self-advocacy skills and provide health and wellness education)
- Provide education, support and consultation to family members, significant others and community members to meet the rehabilitation goal(s) set by the consumer
- Develop individualized and generalized community supports
- Comply with regulatory bodies (i.e., DMHAS, Medicaid, CARF)
- Provide the appropriate number of service contact per DMHAS contract
- Be available to flex schedule to include evenings, as well as weekend and holidays if necessary

- Serve on internal and external committees as required.
- Attend training sessions, workshops, seminars as requested by supervisor, or as needed for professional development, or as required for accreditation or licensing.
- Perform other duties as assigned.
- Regular and consistent attendance is an essential function of this position.
Qualifications
Master’s in Social Work required.
Must be proficient with PCs and Microsoft Office including Word, Excel & Outlook and be able to navigate through electronic health records.
Excellent oral and written communication skills required.
Must have and maintain a valid CT driver’s license if relocated to CT as a state resident more than 30 days ago. Must maintain an acceptable driving record and be insurable at a reasonable rate under the Organization’s auto insurance plan.
Knowledge of Psychiatric Rehabilitation is desirable.
LPC or LMSW in CT required. LCSW in CT preferred. Minimum of 4 years’ experience in a community mental health environment. Minimum of 2 years supervisory experience, in lieu of, a minimum of 3 years in roles demonstrating leadership responsibility.
