Job Description: Psychiatric Unit 5 X 5.5 hour days, first shift - 8:00a-1:30p (M-F) Job Description: The Nurse Navigator is a dedicated position responsible for coordinating discharge planning and case management of patients within the hospital setting The Nurse Navigator provides hands on interaction with aiding patients in conjunction with family members in achieving an effective transition from hospital to post-hospitalization care through assessment, planning, relaying information, and referral to appropriate community agencies throughout the discharge planning process. The Nurse Navigator also provides advice or guidance for any other non-medical problem or barriers to care patients may have that may hinder their rate of recovery or delay discharge and serves as a resource for continuity of care options after discharge. Requirements: Current unrestricted licensure as a registered nurse in the state of Kentucky required. Bachelor's degree in nursing (BSN) required. BLS from American Heart Association (AHA) – required within 90 days of hire. Preferred Qualifications: Three years of Psych nursing preferred. Psychiatric – Mental Health Nursing Certification (PMH-BC) preferred.
