Summary/Objective
The Behavioral Health Patient Navigator functions as a single point of contact for patients in need of resources and assistance in accessing clinical and supportive care services offered within Baptist Behavioral and the community at large.
Essential Job Duties

- Assists patients in understanding their treatment options and the resources available to them within the Baptist system and the surrounding community as well as helping to resolve barriers to accessing primary psychiatric and preventive healthcare service in light of anticipated discharge.
- Establishes and maintains positive working relationships within the behavioral healthcare community, as well as those business in the community at large, that can ensure patients receive the care and services needed.
- Educate patients in regards to where and when and how to access behavioral healthcare services including the roles played by the various outpatient behavioral health resources post discharge to aid in the obtainment of optimal recovery outcomes.
- Assists in discharge planning and provides appropriate linkage with post-discharge care providers, following up with patients to ensure that they received the care and services needed as well as educating patients in the importance of adhering to post care instructions given by providers.
- Connects underfunded/uninsured patients to financial counselors and enrollers when appropriate.
- Maintains appropriate documentation in regards to patient contact, referrals and services provided to patients and is familiar with the P&P governing the use of this information in the Electronic Health Record.
Required Education and Experience
Bachelor’s Degree in Social Work or BS in Nursing
1-3 years of related experience.
Required License/Certifications
Licensed Social Worker or Registered Nurse in State of Texas
