This is an exciting LPN opportunity to work On-Call under the direction of the Hospice RN. You will perform on-call visits throughout the 8-county service area.

Qualifications, Duties, and Perks
Main Duties and Responsibilities:
(Includes, but not limited to)

Under the direction of the registered nurse, provides end-of-life care to patients and families.

Monitor and evaluate medication need and administration of patients, including symptom management, supplies, and equipment needs.

Documents patient visits utilizing the EMR system (Matrix Care).

Communicates progress and needs of patients and families to other interdisciplinary team (IDT) members promptly and professionally.

Assess patient safety and provide education on safety issues to patients and families.

Refer patients to other disciplines as needed.

Educate patients and families on using durable medical equipment (DME) appropriately.

Assist HHA in providing HHA services as needed.

Provide RN Case Manager and triage with daily reports of patient progress.

Deliver supplies for RN Case Manager as needed.

Assists RN Case Manager in preparation of IDT review during team meetings and via reports.

Provide HHA services in the absence of HHA while on-call during visits.

Satisfactorily complete competency requirements for this position.

Under the supervision of an RN, the LPN provides palliative nursing care to patients and families, incorporating psychosocial, cultural, physical, spiritual, and biological components and appropriate nursing intervention as identified in the patient/family plan of care developed by the IDG.

Demonstrates clinical skills regarding application of Hospice Clinical and Administrative policy and procedures such as: pain medication titration, symptom management, and patient/family relationships.

Observes and reports signs of adverse reactions, response to treatments and results of dressing changes.

Consults and collaborates with other professionals to promote quality and continuity of patient/family care.

Nursing documentation displays accuracy, timeliness, legibility, professional terminology, and awareness of legal implications when nursing care is delivered.

Presents concise, pertinent oral and written patient information to the team, respects input from all disciplines.

Patient-care visits assigned by the RN Triage and pre-determined by RN Case Manager on weekends for follow-up visits. Performs routine patient care visits as needed under emergency circumstances only.

Performs all duties within the framework of the Hospice's philosophies, policies, and procedures.

· Performs on-call visits in adjacent counties as requested by the Team Manager or Clinical Director(s).

On-call when needed by special assignment under the direction of Hospice RN.

Performs other job duties as requested.

Requirements
- Valid and current Florida licensure as an LPN is required.
- Minimum of one [1] year experience in a medical/surgical inpatient unit
- Hospice/Home Health experience preferred.
- Ability to physically access any home in the service area, i.e., navigate stairs, narrow spaces, lack of air conditioning and heat, etc.
- Ability to remain calm and troubleshoot/handle patient/family crisis situations.
- Highly organized, with the ability to manage multiple priorities simultaneously and effectively.
- Excellent verbal and written communication skills
- Excellent organizational skills and attention to detail
- Ability to act with integrity, professionalism, and confidentiality
- Ability to make decisions based on the expectations of supervisors.
- Proficient with Microsoft Office Suite or related software including an EMR system.
- Valid driver's license and proof of automobile insurance.
