Job Summary:
The primary responsibilities of this position are to provide skilled evaluation and treatment as prescribed by a physician to patients with communication deficits, swallowing disorders, and/or cognitive deficits; to educate patient or family members; and to function as a member of the rehabilitation team. Responsible for ensuring that care provided to patients is of excellent quality, meets home care rules and regulations, and follows physician’s orders. All employees, if applicable, are expected to adhere to the BSHR Automobile Safety Program per policy.
Essential Functions:
Performs various test procedures to evaluate the structural and functional capabilities of the peripheral oral mechanisms and the functional status of the central language processing systems.
Assesses the type and severity of speech, voice, language, and oral-pharyngeal disorders, and considers the diagnosis, prognosis, pertinent medical history, and home family environment to determine rehabilitative potential.
Identifies deficits based on evaluation and establishes goals for speech/language therapy, with patient/family participation, and in accordance with physician orders; revises goals based on changes in functional speech, motivation, clearing of confusion, and responses to treatment.
Provides direct patient treatment based on the evaluation, the established plan of care, and knowledge of speech/language pathology principles and practices. Such treatment may include but is not limited to the following: alaryngeal speech, articulation, language, voice, fluency training, dysphagia treatment, cognitive training, augmentative communication training.
Measures progress in response to treatment.
Instructs patient, caregivers, and agency staff in activities to be implemented by the patient and/or family as part of a home program.
Collaborates with the nurse, physical therapist, medical social worker, and/or occupational therapist, in the development and implementation of the Plan of care.
Utilizes appropriate facility and community resources to assist in speech/language therapy services.
Provides information and assistance to families in obtaining necessary communication equipment.
Consults with RN, Rehab staff, and/or physician concerning significant changes in patient’s status relative to prescribed treatment and obtains additional orders as needed.
Submits necessary documentation for physicians, medical records, third party payors, and office/department records within the designated time frame. This includes initial evaluation, Plan of Care, goals, and physician orders, clinical notes for each treatment session, non-skilled visit, or canceled visit. Clinical notes regarding pertinent phone calls or conferences with physicians, home health staff, family members, or other sources. Interim patient progress reports for recertification of orders. Physician changes in orders. Discharge summary of services rendered and patient discharge status. Documentation of visits/treatments for billing and statistical reports.
Implements clinical pathway appropriately and is responsible for transporting pathway tool back to the office if assigned as case manager.
Follow clinical pathway plan and documents appropriately on pathway.
Consults with referring physician concerning impending discharge and treatment provided; obtains concurrence with, and orders for, discharge plans.
Schedules patient visits in accordance with the frequency and duration authorized by physician.
Exhibits interest in professional growth and knowledge through attendance at in-service programs.
Serves on home care committees as requested.
Provides assistance in orientation of new employees as requested.
Attends and participates in home health staff conferences, meetings and meets with Rehab Services Clinical Coordinator as requested.
Independently meets productivity standard.
Participates as a team member by performing additional assignments not directly related to the job description when workload requires and as requested by supervisor.
Job Requirements:
1) Masters in Speech/Language Pathology or Communicative Science and Disorder
2) Licensure as a Speech Therapist in the state of VA
3) Completion of Clinical Fellowship Year (CFY) and possession of Certification of Clinical Competence in Speech/Language Pathology (CCC-SLP)
4) 1 year of experience, Preferred work experience with all techniques and methods used in evaluation and treatment, and with the application of anatomy/physiology, and testing methods to treatment, as generally acquired during one or more years’ experience as a clinical speech pathologist.
5) Possess valid driver's license with an acceptable driving record
