The Intake Screener is responsible for triaging new clients to determine if immediate services are needed, or the person may be scheduled an appointment. The Intake Screener provides direct clinical Services to clients of Bridgeway Center, Inc. (BCI). The overall purpose is to provide quality service delivered with dignity and respect for the client. This service is designed to assess and link clients to the appropriate levels of care, improving their overall mental health and generating increased community wellness. The Intake Screener will practice ethically, work with colleagues and clients with respect, and be subject to supervision of his/her activities. The Intake Screener will perform the job in a normal office or community environment.

MINIMUM REQUIREMENTS: Bachelor’s degree from an accredited college or university with a major in Social Work, Psychology, Counseling, or a related human service field. Experience working in direct behavioral health care services preferred. Spanish-speaking or Bilingual applicants are a plus, but not required.

Applicant must be able to pass an Agency for Healthcare Administration (AHCA) 2 / 3Medicaid and Florida Department of Children and Families (DCF) Level II background check, drug screen, and a Tuberculosis (TB) test. Applicant must be at least twenty-one (21) years old and have a valid Florida Driver’s License with no more than three (3) points on their driving record to be insured under BCI’s automobile insurance policy.
DUTIES/RESPONSIBILITIES:
1. Provide support to develop clear, defined goals/objectives for the CCBHC grant from the Disparity Impact Statement (DIS), using the SMART (Specific, Measurable, Achievable, Realistic, and Timely) model.
2. Demonstrates the flexibility necessary to respond to client needs.
3. Demonstrates the ability to evaluate client’s status to identify any danger to self or others and takes necessary steps to assure client safety.
4. Demonstrates the ability to identify client needs for mental health maintenance, abstinence from substances, family support, educational/vocational needs, medical/dental needs, legal assistance, and increasing support systems.
5. Ensures the BCI Registration Packet is completed before initiating the assessment.
6. Ensures the initial screening assessment is completed before referring to services.
7. Makes appropriate referrals based on the initial assessment information.
8. Evaluates diagnosis criteria information obtained in the initial assessment in accordance with DSM-5 guidelines.
9. Ensures the electronic health record includes documentation and documents pertaining to the assessment.
10. Provides crisis intervention services as needed which may include referral for Baker Act Assessment.
11. Knowledge of community resources and assists in linking persons served to the most appropriate services.
12. Educates clients and their support system on alternative options to hospitalization. Provide backup coverage for maintaining the BCI and Behavix wait lists.
13. Demonstrate competency with adding to and removing clients from wait lists.
14. Connect with the clients on the list at least monthly to determine if they are stable and continue to desire services. Completes all state and federal outcome measures promptly and correctly with minimal errors.
15. Maintains compliance with documentation in accordance with BCI Procedures.
16. Documents collateral contacts such as telephone calls, chart reviews, and staffing with other providers.
17. Ensures Consent to Treat is signed by the consumer or the legal guardian. When there is a legal guardian, ensure appropriate court or Power of Attorney (POA) documents are in the electronic health record.
18. Ensures a current/up-to-date Release of Information (ROI), signed by the client, is in the electronic health record.
19. Uses the concurrent and collaborative documentation process.
20. Provides backup coverage for Substance Use and Behavioral Health Groups.
21. Understands and utilizes group processes to facilitate groups.
22. Addresses client rights and responsibilities, including group rules and confidentiality of group members.
23. Documents group therapy sessions in a timely manner and by Bridgeway established formats to include progress or lack of progress toward treatment plan goals and objectives via treatment plan problem number or objective number.
WORK LOCATION: Fort Walton Beach (FWB) WORK HOURS: Mon – Fri; 40 hours per week
WAGE: The wage for this position is $20.00 per hour.
