As the Hospice Physician, you will have the opportunity to collaborate with our clinical teams to contribute to that mission. These passionate team members will depend on you to provide oversight of clinical and physician services, and to be a reliable go-to problem solver.
On an average day, you will
- Collaborate and act as a resource with the IDT/IDG to ensure that the medical needs of the patient are met and provide plan of care oversight
- Assure overall continuity of the hospice medical services to include contractual services
- Assure patients receive appropriate measures to control uncomfortable symptoms
- Certify the patient meets medical criteria for admission based on available diagnostic and prognostic indicators
- Re-certifying patients, for the continuation of benefits at the appropriate level of care
- Communicate with the Medical Director of the SNF or NF, the patient's attending physician, and other physicians participating in the provisions of care for the terminally ill, and related conditions to ensure quality of care for the patient and family.
- Serve as a medical resource for all
- And perform additional, related duties as needed

Qualifications
Qualifications/Desired Skills
- Passion for providing the best possible care to patients and their families
- Excited at the idea of doing work that makes a difference while enjoying the perks of a great work/life balance
- At least 2 years of Hospice MD work experience
- Current license to practice medicine in your state
- Love being the go-to person in the organization
