On an average day as a Social Worker, you will:
- Have the opportunity to collaborate with The Director of Clinical Services and other clinical team members (physicians, chaplains, therapists)These passionate team members will depend on you to provide social services to patients and their families and to be a reliable go-to problem solver.
- Offer short-term individual counseling, crisis intervention
- Assist in providing information and preparation of advance directives
- Assist with funeral planning issues and transfer of responsibilities regarding fiscal, legal, and health care decisions
- Identify support systems available to reduce stress and facilitate coping with end-of-life care
- Identify and utilize appropriate community resources and assess the patient/family's ability to access them
- And perform additional, related duties as needed

Desired Skills/Qualifications
- Excitement for doing work that makes a difference
- Must have an active license in the state you are applying
- Must have a valid driver's license, auto insurance, and reliable transportation for the purpose of traveling to our patients
- One year of hospice-related experience is preferred
- A Masters degree in social work from a school accredited by the Council on Social Work Education is strongly preferred
- A passion for providing care to patients and their families during and after the dying process
- A desire to work in a team environment
