Care Manager Extender Service Definition and Required Components Through Tailored Care Management, Behavioral Health I/DD Tailored Plan beneficiaries will have a designated care manager supported by a multidisciplinary care team to provide whole-person care management that addresses all their needs, spanning physical health, behavioral health, I/DD, traumatic brain injury (TBI), pharmacy, long-term services and supports (LTSS) and unmet health related resource needs.  The Peer Specialist, will assist in meeting the needs of the member, create additional workforce capacity, and allow care managers and supervisors to focus on key tasks for assigned members as well as permit them additional time for members with intensive or complex needs.   Responsibilities include but not limited to the following: •            Performing general outreach, engagement, and follow-up with members; •            Coordinating services/appointments (e.g., appointment/wellness reminders, arranging transportation); •            Engaging in health promotion activities (as defined in the Tailored Care Management Provider Manual) and knowledge sharing; •            Sharing information with the care manager and other members of the care team on the member’s circumstances; •            Providing and tracking referrals and providing information and assistance in obtaining and maintaining community-based resources and social support services; •            Participating in case conferences; •            Support the care manager in assessing and addressing unmet health-related                       resource needs; •            Regular communication and coordinate efforts with Care Manager and Care Manager Supervisor to decrease duplicative and inappropriate outreach to assigned members; •            Other duties as assigned.   Service Type and Program Requirements ·       Person-Centered Thinking/planning ·       Knowledge of using assessments to develop plans of care ·       Knowledge of LOC processes ·       Knowledge of Medicaid basic, enhanced MHSUD, and waiver benefits plans ·       Knowledge of and skilled in the use of Motivational Interviewing ·       Knowledge of and skilled in the use of Motivational Interviewing techniques ·       Strong interpersonal and written/verbal communication skills ·       Conflict management and resolution skills ·       Proficient in Microsoft Office products (such as Word, Excel, Outlook, etc.) ·       High level of diplomacy and discretion is required to effectively negotiate and resolve issues with minimal assistance. ·       Ability to make prompt, independent decisions based upon relevant facts ·       Good organizational skills to prioritize duties and work with minimal levels of onsite supervision to consistently meet deadlines       Staff Qualifications and Role   •            Is at least 18 years of age; •            Has a high school diploma or equivalent; •            Is trained in Tailored Care Management (as described later in this document); •            Is supervised by a care manager at an AMH practice, CMA, or Tailored Plan, and meets one of the below requirements: o   Be a person with lived experience with an I/DD or a TBI with demonstrated knowledge of and direct personal experience navigating the North Carolina Medicaid delivery system. or o   Be a person with lived experience with a behavioral health condition and a Certified Peer Support Specialists. or o   A parent or guardian of an individual with an I/DD or a TBI or a behavioral health condition and has at least two years of direct experience providing care for and navigating the Medicaid delivery system on behalf of that individual (parent/guardian cannot serve as an extender for their family member). or o   Has two years of paid experience performing the types of functions described in the “Extender Functions” section below, with at least one year of paid experience working directly with the Tailored Care Management eligible population.   Individuals who will be able to meet these qualifications, including, but not limited to the following: • Certified Peer Support Specialists; • Community health workers (CHW), defined as individuals who have completed the NC Community Health Worker Standardized Core Competency Training (NC CHW SCCT); • Individuals who served as Community Navigators prior to the implementation of Tailored Plans; and • Parents or guardians of an individual with an I/DD or a TBI or a behavioral health condition (parent/guardian cannot serve as an extender for their own family member); • A person with lived experience with an I/DD or a TBI or a behavioral health condition
