Job includes working most evenings Monday-Friday and some mornings as needed.

Salary: $55,000 - $55,000

Position Summary:

BCS’s Transitional Living Community (TLC), a 30 bed section of the Brooklyn Women’s Shelter in East New York, Brooklyn, was established in 1989. TLC provides homeless, low-income, mentally ill women with temporary shelter and assistance to secure permanent housing. TLC recruits its clients primarily from the assessment unit of the Brooklyn Women’s Shelter. Staff conduct comprehensive assessments to identify women with severe and persistent mental illness, many also having serious personal, social, medical and economic problems. A principle feature of the TLC program is the “milieu treatment” in which women are helped to practice the skills they will need to live successfully outside the shelter system, including pre-vocational preparation.

TLC is a transitional living community for mentally challenged homeless women. Vocational Educational Specialist works with other staff to ensure that clients are prepared for permanent housing, employment and vocational services by facilitating TLC’s onsite Rehabilitation Program, and the facilitation of groups for on and off site activities. The Vocational Educational Specialist conducts and facilitates computer lab training and works with recruited volunteers to conduct various onsite groups and workshops.

Responsibilities:

• Provide supervision to program Case Managers and Case Aides.
• Schedule regular meetings and engage clients to ascertain or update needs.
• Supervise staff in completion of Case Notes and Independent Living plans in CARES. Also, immediately report all placements to ensure entry into CARES within 24 hours.
• Ensure Client Adhere to COVID standards and requirements.
• Assure that all clients have access to primary care physicians and specialized care providers as needed.
• Assume responsibility for collecting health care data on all clients, in collaboration with their primary Case Manager, and enter medical data into the client’s electronic health records.
• Make referrals to BCS Health Home, BCS PROS, and BCS Clubhouse and coordinate services for these mutual clients.
• Participate in case conferences with the TLC team offering clinical observations and recommendations for individualized behavioral strategies.
• Complete Supervisory progress notes, complete psychosocial histories, complete treatment plan reviews and track health-related data using Foothold AWARDS.
• Update client demographic and other data including discharges in AWARDS in a timely manner.
• Schedule appointments for physical exams and meetings with psychiatrist then follow up to obtain results for integrated care.
• Submit 2010E in HRA database.
• Coordinate with the Housing Specialist for connections with Housing Providers and to assure an adequate number of placements from the caseload, with a focus on long term stayers and chronically homeless individuals.
• Coordinate with Housing Specialist to provide at least monthly follow up to placed clients/ aftercare.
• Prepare for case conferences.
• Co-ordinate with Voc/ Ed and Case Manager to assure that service plan goals are being met and entered into AWARDS.
• Assist with development of funding proposals as needed.
• Participate in agency wide groups and meetings as required.
• Perform other duties as assigned by the TLC Program Director.

Qualifications: Required - MSW with a minimum of one year of experience with homeless and/or mentally disabled individuals preferable OR MHC or Masters in a Human Services related field, with three years of experience with homeless/severely and persistently mentally ill populations, preferably in a residential setting.
