What are the responsibilities and job description for the Speech Therapy (SLP) Home Health position at BUCKEYE HOME HEALTH CARE?
Buckeye Home Health Care continues to grow and is looking for a full-time/per diem Speech Therapist to join our team. The area covered is Shelby, Dark, and Miami Counties. This position would offer very flexible hours and a stable caseload.
We are looking for someone who can help improve the well being of our patients and their families and has the heart and skills to work with home bound patients.

Qualifications:
· Ability to perform speech therapy evaluations, routine visits, and OASIS documentation
· Work with other disciplines to give the best care needed for each patient.
· Help decrease the need for re-hospitalization by being actively involved in the patient's plan of care to assess any changes that may be needed.
· When appropriate teach and counsel family members about care and processes for disease/injuries.
· Attend orientation, meetings, and CEUs
Requirements:
· Computer skills; must be able to work within EMR system.
· Must have current Ohio speech therapy license in good standing.
· Reliable auto and current auto insurance.
· Current CPR.
