As a Community Navigator at CAL-PEP, you will play a crucial role in connecting individuals and families in our community with the essential resources and services they need. Community Navigators serve as guides, advocates, and support systems, helping community members access health care, social services, education, and other vital resources to improve their quality of life.
Responsibilities:
Resource Referral
Provide information and referrals to community members, connecting them to available services, programs, and resources that address their specific needs. This may include healthcare, housing, education, employment, food assistance, etc.
Support and Advocacy
Advocate on behalf of clients, ensuring that they receive the assistance and support they require. Help clients navigate complex systems and overcome barriers to access services.
Education and Empowerment
Empower clients by educating them on their rights, responsibilities, and available options. Offer guidance and support to help them make informed decisions.
Documentation
Maintain detailed and confidential client records, documenting interactions, referrals, and outcomes.
Community Outreach
Actively engage with the community to raise awareness of available services and programs, conducting outreach and participating in community events as needed.
Cultural Competence
Demonstrate cultural sensitivity and an understanding of the diverse needs of the community you serve.
Collaboration
Collaborate with other social service agencies, organizations, and community partners to expand access to resources and improve community well-being.
Regular Reporting
Provide reports and updates to the Program Manager or Director of Community Services on the progress and challenges assisting clients.
Qualifications:
● High school diploma or equivalent; a bachelor's degree in social work, human services,
or a related field is preferred.
● Previous experience in community outreach, social work, case management, or a related
role is a plus.
● Excellent communication and interpersonal skills.
● Knowledge of community resources and social service agencies in the area.
● Strong problem-solving and advocacy skills.
● Cultural competence and the ability to work with a diverse range of individuals.
● Empathy, patience, and the ability to build trust with clients.
Job Type: Full-time
Pay: $22.00 - $26.00 per hour
Expected hours: 32 – 40 per week
