About the Position:
The Service Coordinator is responsible for developing and implementing a robust social services program in our HUD subsidized elderly and disabled properties. Every day is different, but the majority of the Service Coordinator time is spent connecting residents to programs, activities, benefits, and services that will support our residents’ quality of life and respect their independence. If you are someone who can promote a positive social climate, empower residents to meet their goals through education, training, and accessing services, and love to ask "why", then you may be the one we are looking for!
This is for a 198-unit property in Van Nuys, CA designed exclusively for low-income seniors. The position is full-time, 40 hours per week.
Responsibilities:

Educate and link residents, families, and staff on and to available community resources, including service availability, application procedures, and resident rights, both individually and as a group

Develop a diverse community network to support on-site programs and services

Engage residents in the Service Coordinator program via group and individual activities and services

Aid residents in applying for benefits such as food stamps, Medicare, utilities programs, Medicaid, and other community-based benefits

Act as liaison between residents, management, and social service agencies

Assess and advise residents and their families of the services which may be helpful/necessary to maintain a quality, independent living lifestyle.
Qualifications:

Prior related case management experience required

Prior experience working with a Senior population is helpful

Computer literacy is required

Must enjoy working independently and possess excellent organization and time management skills
