GENERAL SUMMARY:
Memory Care Assisted Living Nurse – LPN is responsible for ensuring the general well-being of residents through the delivery of high quality services.
The Memory Care Assisted Living Nurse – LPN is responsible for quality care and for providing a “system of care” for residents in a manner and environment that promotes dignity and respect in recognition of each resident’s individuality.
PRINCIPAL DUTIES:
Essential Job Duties:
1. Provides care management services to residents, monitors medication regimen and works with the pharmacy to ensure the appropriate handling, dispensing, and destruction of medications occur.
2. Assist with admission and orientation of new residents to the community, ensuring satisfactory integration into the community.
3. Identifies safety hazards, and institutes corrective action in a timely manner. Shares in the responsibility for providing and maintaining an environment that is conducive to the safety, physical and emotional well-being of residents and staff.
4. Maintains communication reflecting the health and mental status of residents, and reports in a timely manner any changes in condition to his/her physician.
5. Participates in investigations into resident incidents involving clinical issues with department manager.
6. Assists in the coordination of residency termination, discharge and transfers.
7. Observes and will assist in direct resident care, as needed.
8. Conducts audits of med carts on a weekly basis to ensure appropriate medication administration. Conducts monthly audits of MARS and TARS.
9. Participates in assessment of residents both new to the community and when required for traffic/community navigation purposes.
Personnel
1. Works with Resident Attendants and Care Partners including training and onboarding, performance evaluation, and coaching/motivation of staff to ensure the provision of high quality services to residents in a manner that is consistent with the assisted living philosophy. Utilizes appropriate disciplinary measures as necessary; counsels and advises staff members. (Done in conjunction with department manager.)
2. Coordinates and conducts clinical training and orientation of new staff.
3. Determines needs, develops materials, and conducts in-service training for staff on clinical matters.
4. Promotes and facilitates interactive problem-solving with staff.
5. Responds to the medical alert call system, assessing each situation as needed. Notifies 911, department manager, physicians, family and staff as needed.
Organizational
1. Serves as a liaison between the community, residents, families and physicians.
2. Ensures compliance with all federal, state and local regulations.
3. Initiates and monitors continuous quality improvement program.
Other Job Functions
1. Maintains an environment that acknowledges and supports each resident’s rights to make choices regarding their surroundings and care.
2. Assists in implementation of the community’s marketing plan.
3. Consistently demonstrates adherence to Infection Control policies and procedures in all aspects of performance.
4. Coordinate with the food and beverage staff to ensure that nutritional and therapeutic diets are served to residents.
5. Perform work duties and responsibilities as assigned by the department manager.
6. Takes after hours calls/texts from staff regarding resident care inquires that require immediate attention.
7. Assists with setting up, maintenance, and troubleshooting the Med Alert and Wanderguard Systems.
8. Responds to inquiries from guests, visitors, sales representatives, etc.
9. Completes work orders for broken or malfunctioning equipment and forwards to supervisor.
10. Recommends improvements and offers suggestions as appropriate.
11. Utilizes time in efficient manner, completing duties in timely fashion.
12. Maintains skills and competencies utilized in work areas.
13. Performs other duties as assigned.

- Utilizes proper use of body mechanics and personal protective devices.
- Meets or exceeds internal and external customer service expectations and promotes the continuous improvement philosophy of the department and community.
- Uses equipment, services and supplies in a cost-effective manner.
QUALIFICATIONS:
Education Must be a registered nurse or licensed practical nurse graduate of approved school of nursing and possess a current nursing license to practice in the state of Michigan. Must be able to think and function as an “in-charge” person. Must have the ability to read, write and speak English.
Experience Must have supervisory and geriatric nursing experience and sensitivity to the interest of and understanding of the physical, mental, social, spiritual characteristics of aging, while maintaining an appreciation of the residents’ skills and be capable of relating to a variety of people and personalities.
Considerable initiative, judgment, and leadership skills are also required. Must have demonstrated ability to direct and manage the skill levels required.
