Job Summary:
Responsible for coordinating the Advanced Valve Disease/Structural Heart Disease clinic and all aspects of patient journey between Kent Hospital and Brigham and Women’s Hospital. Responsible for coordinating and maintaining surgical schedules for the practice.

Minimum of 2-4 years of experience in a healthcare environment.

Duties and Responsibilities:
- Be the point person for patient/families to stay connected to the Kent/BWH process
- Provide support for Kent/MDs/BWH (Structural Heart PA Coordinators, Billing Coordinator, OR schedulers, Post op schedulers and Structural Heart Program Coordinator)
- Work closely with office staff for scheduling and tracking patients at Kent Clinic
- Coordinates with hospitals to schedule and arrange all patient pre-op testing, medical clearances, etc.
- Pre-clinic:
- Obtain records/imaging CDs and reports from both Kent and outside facilities; ensure the prep sheets are obtained before clinic for clinic providers prepared by BWH Structural Heart PA
- Review clinic to determine each patient is scheduled for clinic in coordination with prep sheets
- Share patient list with BWH billing coordinator for billing purposes to align with post clinic patient review
- Confirm Out of Network benefit insurance status for coverage of surgery/intervention at BWH.
- Inform patients about the BWH component of their care including description of the facility, travel and expense considerations. Assist in BWH registration process including assisting them over the phone
- Inform relevant physicians of confirmed visits and procedures
- Share & upload all clinic forms with BWH valve team
- Manage and transfer records from treating physicians to BWH for incorporation into BWH Epic.
- Maintain a registry of all treated patients to review with Director
- Coordinate with Clinical Research Coordinators if patients are eligible for a Clinical Trial
- Record and monitor patient scheduled events and contact patients to confirm and assist with protocol compliance. Such activities include but are not limited to:
- Work with BWH post op scheduler(s) to determine post op/follow up visits (BWH will also reach out and share details to schedule patients)
- Confirm, book and track the scheduling of 30 day and 1 year Structural Heart follow up appointments within specific window
- Arrange and confirm for any imaging/testing (ie CTs, echos, COVID testing etc) that needs to be performed locally
- Follow up on this imaging results and transfer to BWH in a timely manner
- Schedule testing need or follow up with patient where/when they will have testing at outside facility
- Keep track of patients and dates of testing and when ready to schedule at BWH
