CareAparent is seeking new graduates and experienced Physical Therapists (PT) to perform home health visits. As a home care Physical Therapist, you will be an integral member of a multi-disciplinary that provides rehabilitative care to clients, allowing them to live independently and age in place safely. Our PTs frequently travel in and around the Twin Cities metro area from one client home to another. We are currently seeking PTs to service the following areas:
- Apple Valley
- Bloomington
- Hastings
- Maple Grove
- Maplewood
- Minneapolis
- Saint Paul
- Woodbury
Duties Include:
- Evaluate and treat clients suffering from physical disability due to injury, disease, or surgery in a home care environment
- Provide treatment for clients using a variety of methods including exercise, hands-on therapy, stretching maneuvers, and equipment to ease clients' pain and help them increase their mobility
- Test, measure, and assess clients' current condition and abilities
- Assist patients with all phases of Physical Therapy treatment and techniques to include exercise instruction, manual therapy techniques, activities of daily living, functional/total body progressions, modalities, and a fall prevention program
- Establish treatment plans specific to the client's specific needs that allows for excellent client outcomes
- Make recommendations for devices or tools that might improve the client's quality of life
- Work cooperatively and consistently with physicians, case managers, nurses, occupational therapists, and caregivers
- Provide training to team members as needed
- Request interventions to help avoid unnecessary trips to the emergency room or readmissions
- Monitor, document, and report clients' progress
- Will delegate tasks to PTAs and will perform supervisory visit of a PTA every 6th treatment session
- Will perform SOC visit when no nursing services are required
If YOU have the following, CareAparent wants to hear from you:
- Graduate of an accredited Physical Therapy program
- Current and valid PT license in Minnesota
- Medicare experience preferred
- Ability to work flexible hours, including weekends and on-call rotations
- Valid Driver's License and reliable vehicle
- Demonstrated ability to work autonomously
