Position Summary

The Intake Coordinator is responsible for all aspects of the initial assessment and referrals for clients and their family following intake. Intake Coordinator is responsible for interviewing, gathering data for a comprehensive psychosocial assessment, forming a clinical diagnosis, and completing the interim service plan.

Essential Duties

  • Demonstrates ability to maintain confidential/sensitive information.
  • Requires advanced knowledge of Microsoft Office Suite computer applications, aptitude for learning computer software applications and the ability to instruct others in the use of various software applications.
  • Maintains clinical records in a timely manner according to standards.
  • Conducts a comprehensive psychosocial assessment, makes treatment recommendations, summarizes primary presenting issues, identifies child/family strengths, and formulates a diagnosis based on this initial interview.
  • Builds rapport and credibility simultaneously with varying members/age groups. Relates to variety of individuals with warmth, empathy, acceptance, and humor.
  • Develops interim service plans that identify the next steps taken after intake by the provider and person/family and what issues are to be addressed for ongoing assessment.
  • Provides effective crisis assessment including the following of program triage protocol, arranging for emergency psychiatric evaluation/hospitalization as indicated by protocol, provides for follow-up, normalizes situation, discusses options, and handles situation calmly.
  • Formulates, conceptualizes, and elicits problem statements from the potential client.
  • Participates as a team member, utilizes supervision, and able to accept peer support in the treatment planning process.
  • Redirects if no behavioral health problem exists, refers client to an appropriate community resource.
  • Conducts educational activities that promote client understanding of treatment services, including therapy, behavior management, psychiatric evaluations, medication, diagnosis, parenting skills.
  • Performs duties with knowledge and skill in group processes, establishes group commitment to common goals, and establishes an environment of mutual respect and understanding.
  • Performs duties using knowledge and skill in assessment and diagnosis utilizing DSM-V criteria.
  • Understands human development and family life cycle in children and families and demonstrates application of this knowledge in assessment and diagnosis.
  • Attends all intake department staff meetings, participates in improving admissions procedures, and assesses the quality and effectiveness of intake services.
  • Participates in in-service training and educational opportunities outside the agency as required and as time and resources permit.

Requirements

  • Bachelor’s degree and work experience in the behavioral health field preferred; OR Non-related bachelor’s degree with four (4) years of full-time experience in behavioral health field.
  • Proof of COVID Immunization.
  • CPR/ First Aid certification required.
  • Must have good verbal and interpersonal skills to interact with clients, visitors, and coworkers.
  • Must be organized and able to relate to diverse populations.
  • Must have excellent writing and record-keeping skills.
  • Demonstrate excellent judgment and ability to solve problems.
  • Strong attention to detail and exceptional organizational skills
  • Bilingual in Spanish preferred.
  • Must be able to meet training and organization requirements for the position.

  • Must possess or be able to qualify for and maintain a valid Arizona department of public safety level one fingerprint clearance card.
