House Calls Supervising Physician - Castell

Job Description: As part of the Castell House Calls Team, Physician will provide supervision of two APP’s who provide home-based primary care services and will lead the interdisciplinary clinical efforts House Calls teams. The goal of this service is to improve the care of patients with complex medical complexity and/or social needs who have recurring avoidable utilization. Scope Clinical work is supervising two APP’s who perform home visits, leading interdisciplinary care coordination meetings of the House Calls Team, and performing in-home patient visits when needed. This work will require ½ day per week committed to House Calls independent from time spent treating or coordinating patient care in the physicians clinic. Physician shall not request compensation for House Calls services that are provided concurrently with any other compensated service. IHCHS will not compensate Physician for House Calls services provided concurrently with other compensated services. This role will report to the Castell Medical Director for Home Services Essential Job Duties Provide direct supervision of two APPs in accordance with Intermountain Medical Group supervision requirements, ensuring evidence-based clinical care standards are established, communicated and met. Promote APP autonomy with guidance Assist with workflow specific to House Calls Be available to answer medical questions/concerns. APP’s in the House Calls treat patients with complex medical and social history. The supervising physician must be available to assist APP’s in formulating appropriate treatment plans for patients when needed. Provide dedicated time to each APP for case review and education Promote continued learning and being a better clinician Lead House Calls Team interdisciplinary care coordination meetings weekly. Participate in home visits to perform patient care when needed. Report to the Castell Home Services Medical Director and participate in regular check-ins to review key process indicators and ensure continued success of the House Calls Program Minimum Requirements Medical Doctor or Doctor of Osteopathic Medicine Degree. Education must be obtained through an ACGME or AOA accredited institution, will be verified ABMS or equivalent AOA Board Certification. Active Utah Medical Licensure, or in process of obtaining licensure, is required Willingness to receive and regularly participate in leadership training At least three years of clinical experience Effective verbal, written and interpersonal communication skills Experience with change management with the ability to provide leadership in the adaptation and implementation of new processes and / or technology that enhance safety 1 year experience in supervising APP clinical work Preferred Qualifications Training and experience in completing primary care home visits Physical Requirements: Physical Requirements Interact with others requiring employee to verbally communicate as well as hear and understand spoken information. Operate computers, telephones, office equipment, and manipulate paper requiring the ability to move fingers and hands. See and read computer monitors and documents. Remain sitting or standing for long periods of time to perform work on a computer, telephone, or other equipment.
