Under direction, the Licensed Practical Nurse (LPN) provides direct nursing services and works closely with Registered Nurse, physician, and treatment team at our sub-acute detox facility. LPN will interview and evaluate clients to assess presenting problem and physical condition. The LPN will administer oral and injectable medications and monitor and evaluate the effectiveness and side-effects. The LPN will consult with the client’s team members and client’s family to develop and implement a plan of care, and to determine best treatment course. The LPN will maintain case notes and records of services provided to clients including progress notes, aftercare and discharge summaries.
Education Requirement: Licensed Practical Nurse with a minimum of two years’ experience, preferred in a drug and alcohol setting. Valid Ohio driver’s license with an acceptable driving record and current vehicle insurance.
Licensure Requirement: Licensed Practice Nurse
