POSITION SUMMARY
The Client Care Coordinator is a multi-faceted role with duties that include but are not limited to assisting and connecting clients to appropriate entitlements, input client level data into Contractor Data Collection System (CDCS) and ensure priority access to services.
GENERAL DUTIES AND RESPONSIBILITIES:
- Client Care Coordinators shall be the point of contact when communicating with referral sources and provide information as requested.
- Ensure all written correspondence meets CSSD timeliness standards, contain accurate information and other requirements reflected in this program.
- Client Care Coordinators shall actively engage and assist eligible clients in gaining public insurance, DSS application process and connection to entitlements such as Medicaid / Medicare. Client Care Coordinators must be intimately knowledgeable with DSS regulations as a means to matching clients’ needs and maximizing third party reimbursement. Client Care Coordinators will begin working with eligible clients at initial appointment and reengage client if she/he loses benefits.
- Provide timely and accurate, typed communications with referral source (i.e. assessment results), progress reports, notification of missed appointments.
- Assist in the scheduling of evaluations or any other services within CSSD timeframe requirements.
- Assist program staff in the completion of timely progress and discharge reports.
- Client Care Coordinators shall be responsible for data entry; data quality review checks, ensuring data timeliness and accuracy and use CDCS data outputs when making operational decisions.
- Update Reports for missed appointments with regards to CSSD timeliness standards.
- The Client Care Coordinators are responsible in engaging and motivating client to attend treatment after each missed appointment via telephone, letters or other means.
- Assist with substance use testing and reporting results.
- Client Care Coordinators must provide notification to CSSD should there be more than a two week wait list for CSSD clients accessing any service.
- Assist with the implementation of the Working Alliance Inventory Short Version (WAI-SV) including forwarding WAI-SV surveys to CSSD.
- Other duties as assigned
PQI FUNCTIONS:
- Ensure the consistent accurate collection of the data
- Identify patterns and trends for program
- Use results of data to inform decision makers of trends that may impact services
Qualifications
Significant experience in the human service field. Bachelor’s degree in psychology, social work or related field preferred. Bilingual/Bicultural ability may be required depending on population served.
Key skills possessed include:
• Excellent interpersonal skills and ability to engage consumers from diverse social and ethnic
backgrounds
• Strong organizational skills to handle varied duties and responsibilities
• Strong computer skills including Word and Excel
