About The Role
- Assesses the psychosocial status of patients and families/caregivers related to the patient’s terminal illness and environment and communicates findings to the RN and other members of the interdisciplinary group, wither verbally or through documentation. Provides an assessment in the patient’s identified residence and provides assistance when the residence is not safe and another plan is required.
- Carries out social evaluations, including family dynamics, caregiver abilities, communication patterns, high risk for suicide, neglect or abuse and plans intervention based on evaluation findings. Counsels patients and family/caregivers as needed in relationship to stress, and other identified coping difficulties. Provide crisis intervention when necessary. The goal is to provide education and support in order to prevent a crisis.
- Coordinates respite stays as per family/caregiver needs, facilitating the review of medical records and all that is included in the preparation of and follow-through of the respite stay.
- Provide bereavement counseling.
- Educates patients on, and assists in, preparation of advanced directives and durable power of attorney for healthcare, if not previously executed, and only if the patient is cognizant.
- Provides information and referral services to organizations to patient’s/families/caregivers regarding practical and environmental needs, i.e., private duty agencies, Medicaid programs, Meals on Wheels, Elder Law, etc.
- Maintains clinical competency.
- Assists in coordination of care with personnel at nursing facilities or patients’ other living settings.
- Participates in the orientation of hospice personnel.
- SW participates in the planning of the live discharge of the hospice patient.
- SW provides the emotional support of anticipatory and cumulative grief/loss depending on the wants/needs of the patient/family.
Minimum Position Requirements::
- Demonstrate a passion for the mission of Catholic Community Health
- MSW preferred.
- Ability to work effectively in an environment that is very diverse
- Must possess a valid Kansas or Missouri Driver’s license, dependable transportation, current vehicle insurance, and a verified safe driving record.
- May be exposed to infectious diseases
- Must have the physical ability to make visits to a variety of environments that may include walking on rough terrain and climbing stairs
- Must have physical ability to effectively communicate with others
- Must have the physical ability and appropriate licensure for driving
Salary : $50,000 - $60,000
