What are the responsibilities and job description for the RN position at Catholic Health Home Care?
The Catholic Health Home Care Field RN will assess, plan, develop and implement with the patient or responsible caregiver a comprehensive home health plan of care for nursing and identifies patient needs for other disciplines and services. The RN case manages, coordinates and evaluates the plan of care with therapeutic, social service, pastoral care, home health aide services, other organizations and physicians for an assigned caseload. The RN instructs patient or responsible caregiver in nursing care, disease management, health promotion as directed by the physician's plan of treatment and consistent with Agency regulatory and professional standards.
MINIMUM QUALIFICATIONS: Minimum of one year of recent hands on experience as a Registered Nurse. BLS required. Acute care experience in medicine and surgery in a hospital setting strongly preferred. Candidate should ideally have Electronic Medical Record experience.
