This Home Care Social Work position will cover the North & South Forks of Long Island.
Position Summary:
The medical social worker provides non-medical patient care encompassing: adaptation to illness and/or disability and treatment, evaluating psychosocial discharge planning, facilitating the use of services offered by other community agencies, crisis intervention, mental status examinations, supportive counseling with terminally ill patients and financial management.
Responsibilities:
- Responds appropriately to the special needs of each group served specific to age, language, gender, culture, religion/spirituality, economic background, education or literacy, developmental or cognitive disabilities, physical and developmental disabilities.
- Employee aids in the orientation of new employees
- Provides social work consultation to Home Health Agency staff as indicated and requested.
- Provides short term and long term casework services to patient and /or their families.
- Responsible for intake and screening patients referred for social work services and organizes workload to ensure appropriate and maximum social service to patients and families.
- Maintains records and patient charts, writes reports and summaries on all patient visits, problems and resolutions, completes all forms required by government regulations and hospital and department guidelines.
- Professional social work practices, including understanding of medical social problems and special resources for same. Provides financial management as needed.
- Advocates for patients in securing services.
- Represents the Home Health Agency and Hospital at appropriate meetings and conferences within the hospital and community.
- Attends continuing education programs on-site and off-site
Requirements:
- Master's degree in Social Work from an accredited college or university
- One year experience in the medical field
- Registered Professional Medical Social Worker in NY State
