Work Performed:
- Comply with all policies and procedures set out by the Quality Home Staffing / Community Based Home Care Employee Manual provided at orientation.
- Position is responsible for providing services as assigned by the RN Field Supervisor in accordance with the individual's approved service plan.
- The Certified Nursing Assistant I is required to report any deviations in an individual's functional or medical condition to the appropriate agency personnel.
- Services must be provided during the assigned schedule for each individual.
- Deviations from the assignment including, but not limited to the following must be reported to the agency
- Hospitalizations, Rehab, or Facility placement
- Incidents, accidents or other events that are not aligned with the individual's plan of care.
The Certified Nursing Assistant I must be able to demonstrate competence with assigned ADLs and IADLs or special tasks and perform these tasks in line with the plan of care.
- Tasks may include, but are not limited to:
- Bathing (assist, bed bath, tub bath, shower, sitz)
- Dressing / Undressing
- Mouth care
- Skin care (non-prescription)
- Hair care
- Nail care (does not include nail care for clients with Diabetes)
- Mobility
- Turn and position
- Transfer
- Use of lift
- Assist with ambulation
- Range of motion
- Toileting
- Bedpan/urinal
- Bowel/bladder training program
- Collect specimens
- Perineal / catheter care
- Apply/Remove Condom Catheter
- Eating
- Meal preparation according to client's dietary needs or restrictions, including forcing fluids and restrictions.
- Assist, monitor or feed the client, according to level of care.
ADLs may include but are not limited to:
- Cooking
- Cleaning
- Shopping
- Transportation
- Escort
Special Tasks
- Vital signs
- Temperature
- Pulse
- Respiration
- Blood Pressure
- Height and Weight
- Application of heat/cold
Preventative intervention to prevent decubitus ulcers
- Apply TED hose, as instructed
- Deep breathing/cough techniques
- Remove EKG leads
- Companion or Sitter services, as assigned in the care plan
In addition to the above tasks, the CNA I may be required to provide other tasks that are required by client's needs and level of care (proof of competence require prior to services rendered.
- All tasks must be performed with Infection Control in mind. These precautions may require masks, gloves, gown.
- Handwashing and other appropriate precautions should be done before and after tasks.
- Assure that the home is safe from obstacles that may cause falls ● If additional services are requested, the CNA I must notify the RN Field Supervisor for further instruction.
- The Certified Nursing Assistant I may not negotiate additional tasks, service location, or schedule changes with the individual. The office must be notified for any alterations, failure to do so may result in termination.
- Be present for Supervisory Visits to be performed by RN Field Supervisor as required.
- Accurately report time and attendance by branch office.
- Time and attendance must be logged in via timesheets, or EVV, as required by payer or Quality Home Staffing.
- Required documenting and reporting as assigned by RN Field Supervisor ● Immediately report concerns of Abuse, Neglect or Exploitation to the office ● Cell phones may not be used during assignment, unless approved for EVV services, as instructed
- Report planned absences to the office as soon as possible so the scheduled outage may be reassigned to another CNA
- Be an advocate for individuals needing other community services or a high level of care.
- Dress appropriately and professionally.
- Wear nametag at all times during assignment
