JOB PURPOSE:
Travel between Queens, Brooklyn, and the Bronx
The Clinical Manager partners with the Senior Clinical Director to positively impact the continuity and delivery of care in the community.
JOB RESPONSIBILITIES:
- Monitors care delivery for the agency’s clients.
- Reviews, investigates, completes, tracks, and trends incident/complaint reports.
- Ensures accurate completion of incident/complaint reports and submission to regulatory bodies within required timeframes.
- Follows up with Physicians, clients, caregivers, and contracts for daily service issues.
- Supervise field clinical staff.
- Conducts client and employee satisfaction surveys.
- Makes recommendations for action plans to resolve complaints.
- Gathers and analyzes data to evaluate the organization's performance in quality management and improvement.
- Develops and implements quality and performance improvement plans and initiatives.
- Works with the team to develop internal benchmarks for success.
- Serves as a resource for staff and caregivers with regard to compliance programs.
- Assists with training/orientation of caregivers.
- Compiles monthly reports and statistics in preparation for Committee meetings, attends, and reports such information to the Committee utilizing Microsoft Excel and PowerPoint.
- Completes and submits weekly/monthly contract compliance reports ensuring compliance with required deadlines.
- Review reports from scheduling software daily.
- Reviews and resolves conflicts in services with other agencies.
- Conducts monthly audits of client/PA records for LHCSA and FI.
- Meets with supervisor regularly to inform about issues as they arise.
- Other duties as assigned.
- Develops and maintains positive “customer” relationships (“customer” is defined as co-workers, participants, visitors, vendors, and supervisors).
- Communicates with customers in a caring, helpful, considerate, and culturally sensitive manner.
- Demonstrates a thorough understanding of customer needs and wants and attempts to anticipate and meet customer needs.
- Suggests and implements customer-centered service improvements.
- Utilizes service recovery techniques to resolve issues quickly and to the customer’s satisfaction.
SALARY RANGE:
Minimum: $118,000.00
Maximum: $122,000.00
QUALIFICATIONS:
Education:
- 2-4 year’s experience in the home care healthcare field preferred
- Bachelor’s Degree in Nursing required
- NYS Active Registered Nurse License, without restrictions
Other:
- Must have the ability to maintain confidentiality and strong attention to detail while working in a fast-paced environment.
- Proficient in Microsoft Office Suite: Word, Excel, and PowerPoint
- Knowledge of federal and DOH regulations applicable to Home Health Care is required.
- Excellent communications skills: written and verbal
- Excellent data analysis, organization and relationship building skills.
Job Type: Full-time
Pay: $118,000.00 - $122,000.00 per year
