HealthSource of Ohio is seeking to hire a Diabetes Educator who is passionate about providing quality healthcare to our patients and communities. We are looking for Friendly, Accessible, Credible individuals to join a great team.
Responsibilities:
- Educates the patient in necessary lifestyle changes that improve the chronic condition of diabetes which may include healthy eating, exercise, medication management, glucose monitoring, problem-solving, coping skills, and risk management
- Assists in the diabetes education program that delivers up-to-date educational materials and supports individuals across the chronic disease spectrum
- Ability to be self-motivating and work independently with little supervision.
- Possess a comprehensive background in chronic disease management especially Type 1 Diabetes, Type 2 Diabetes, and Gestational Diabetes, or willing to learn more
- Travel to multiple locations
Qualifications:
- Registered Nurse or Registered Dietitian required
- Certificate in Diabetes Education or willingness to obtain this certification
- 3 months – 1-year experience required
