Join the Genesis family as a Social Services Specialist where you will provide patients/residents with the highest practical level of physical, mental, and psychosocial well-being and quality of life. You will accomplish this by collaborating with patients/residents, their family members/significant other and other nursing center staff to identify patient/resident strengths, social, emotional, and mental health needs and then provide, develop, and/or aide them in accessing those services.
Position Highlights
*Complete a comprehensive assessment for each patient/resident that identifies social, emotional, and psychological needs and strengths.
*Assess each patient/resident for discharge to ensure an appropriate discharge plan is formulated.
*Educate nursing center staff on the role of the Social Services and the psychosocial needs of the patients/residents and their families/significant others including the problems of aging and disability.
*Conduct patient, family, and staff interviews to ensure Care Area Assessments are completed in accordance with regulation.
*Participate in the development of an interdisciplinary plan of care for each patient/resident that identifies the psychosocial needs/issues of the patient/resident, the goals to be accomplished for those needs/issues, and the appropriate Social Services interventions.
*Deliver therapeutic interventions to assist patients/residents in coping with their transition and adjustment to a long-term care facility including the social, emotional, and psychological needs.
* Facilitate short-term supportive counseling and education to patient/resident and family members/significant others to assist in their understanding of placement and nursing center issues and make referrals to the appropriate Social Service agencies when needed.
