Job Description: CENTERSTONE POSITION DESCRIPTION Position Title: Therapeutic Mentor-Community Action Treatment (CAT) Team Manatee County Position Summary: Will provide mentoring and supportive services for children ages 5-17 with a mental health and/or co-occurring disorder as a part of the enhanced Children’s Community Action Treatment (CAT) Team which serves clients and their families in Manatee County. Will develop positive relationships by providing guidance and facilitating interactions and services for children and their families. Will continuously observe children’s behaviors and family interactions and provide vital feedback to other team members. Able to work independently and as a team. Ability to work well with community partners and in a team environment is essential. Services are provided in the community. MINIMUM REQUIREMENTS: High school diploma or equivalent. Two years’ experience working with children and their families. Valid Florida driver’s license with a safe driving record. Special Skills, Knowledge and Abilities: Knowledge and understanding of the characteristics of children with mental health issues, their families, and family systems. Willingness to work on a comprehensive treatment team. Highly motivated, self-directed, willing to work a variety of hours, including evening and on-call. Essential Functions: Reports to work as scheduled on a daily basis to perform job duties to facilitate the development of therapeutic relationships with clients and to facilitate consistent client services. Provide support to child and family as a role model teaching life skills, positive discipline, parenting skills, financial responsibility and homemaking skills. Identify child and family strengths, challenges and weaknesses. Participate in the treatment planning process. Conduct on-going observation of behavior, mental status, signs and symptoms mental health disorder, physical and dental health, substance use, and educational, social, developmental and family functioning. Report observations during bi-weekly meetings. Act as liaison with the school system, community agencies and families to maintain coordination of services and advocate for child and family’s rights and needs. Assist families with developing natural support systems. Provide on-call services during nighttime and weekend hours as assigned. Provide transportation, tutoring and respite services to children and families as assigned. Will engage clients in weekly, at minimum, community outings to develop and enhance age appropriate social skills Will meet productivity requirements of at least 70% as set by management Performs other related duties as assigned by supervisor. Work Environment/Physical Demands: Services are conducted primarily in the community, spends time in outpatient behavioral health settings and operates in a professional office environment; can be exposed to various health conditions. While performing the duties of this position, the employee is regularly required to observe treatment environments and to use speech and hearing to interact with others in person, via telephone, using computer technology and in writing. Routine days and hours are flexible, minimum 20 hours per week.
