Job Description:

“Centerstone is currently requiring that all staff provide proof of a COVID-19 vaccination by October 1, 2021, for leadership positions, and November 1, 2021, for all other staff. Position Summary: Ensures continuity of care to clients through case management services, specifically in mental health. Provide and link clients with the needed resources to improve the client's quality of life. Provides court coordination for those with criminal justice involvement.

Minimum Qualifications: Bachelor's Degree in Social Work, Psychology, or related field. Also requires knowledge of community services. Experience in a mental health setting and previous case management experience preferred. Strong communication, ability to work on a team and time management skills necessary. Valid Florida driver's license with a good driving record and valid auto insurance.

Preferred Qualification: Two years' experience in a mental health setting and previous case management experience preferred.

Special Skills, Knowledge and Abilities: Knowledge of community services. Highly motivated, self-directed, and some flexibility with work hours. Excellent written and verbal skills. Willingness to work on a comprehensive treatment team. Ability to build professional relationships in the community and networking.

Essential Functions/Job Duties:
1. Complete jail assessments and referral to appropriate treatment court programs within given time frame. Perform intakes and orients new clients to the program guidelines.
2. Complete necessary certifications for training facilitation and promote and schedule trainings in the community. Assist with Crisis Intervention Training and other community based trainings.
3. Provide direct services to clients to teach behavioral symptoms and management techniques and to promote personal growth.
4. Refers, advocate and link clients to additional services and resources in the community as needed. Act as liaison and consult with community agencies and judicial system to maintain coordination in the treatment process.
5. Provide ongoing assessments of client's mental status; e.g. symptoms and response to treatment, drug screens.
6. Participate with team in providing treatment, education and services in the areas of employment, activities of daily living, leisure time, housing, substance use, symptom-behavioral management and coping with stress.
7. Inputs clinical documentation into Avatar on a daily basis and in a timely manner for all services rendered directly with or on behalf of clients. (must be documented by the next business day)
8. Completes all required agency (SOAR) and department trainings and related duties as assigned by supervisor.

Work Environment/Physical Demands: This position operates in both an office environment and community based settings. Duties of this job are often provided in the place where the client resides or needs the service. This position may be subject to exposure to individuals who engage in maladaptive behavior.

Position Type, Expected Hours of Work and Travel: This is a full time, 40 hour position. Typical hours of work are Monday through Friday- days. This position requires flexibility in work hours to include some evening and weekend hours as needed to meet the needs of the clients. Frequent local travel is required. On call rotation is required.
