What are the responsibilities and job description for the Licensed Practical Nurse position at CFS?
Position Summary: The nurse is responsible for providing education to youth/adolescents, adults, and guardians regarding mental health services, systems, medications, and side effects. The nurse will also act as a liaison between the Agency and various organizations (e.g. pharmacies, primary care) within the community. This position functions within the outpatient mental health clinic.
Major Responsibilities/Activities:
- Meet with clients receiving services from the clinic psychiatric provider to collect vital signs, weights, review medication benefits and side effects
- Assure there are informed consents for medications
- Provide any information necessary for the proper dispensing of medication at home, and when needed, at school
- Communicate any medication, behavioral or primary health concerns to psychiatric provider, guardian (as applicable) and client's therapist
- Document any concerns in the client's chart and monitor as needed
- Complete prior authorizations with insurance companies to ensure clients receive appropriate medications as determined by the psychiatric provider
- Return phone calls from pharmacies to clarify questions regarding scripts, refills, and other inquiries
- Review all Laboratory test results and discuss any concerns with the psychiatric provider
- Work collaboratively with mental health therapists regarding behavioral health concerns and outcomes
- Ensure completion of appropriate documentation in all clients' charts as deemed appropriate
- Demonstrates appropriate communication skills, both written and verbal
- Must participate in various program committees i.e. Special Incident Review, Case Conferences, and Consumer Advisory as necessary
- Shows respect and courtesy in all interactions and communication internal or external to Agency
- Interest, knowledge, and support for the Agency’s mission, values and philosophy
- Problem solving and conflict resolution skills in both organizational and interpersonal matters
- Supports and communicates with supervisor and coworkers in a cooperative manner
- Must maintain and submit all of the required documents and comply with all aspects of the Vehicle Safety and Usage Policy
- Models and practices sensitivity, fair treatment and acceptance of diversity in all interpersonal interactions
- Participates and contributes to program Quality Improvement process
Minimum Requirements:
- Spanish speaking preferred
- L.P.N. from a recognized accredited school of nursing
- Two years of nursing experience is preferred
- Ability to relate well to children and other staff members
Schedule:
Monday: 8:30am - 5:30pm
Tuesday: 8:30am - 5:00pm
Wednesday: 8:30am - 5:30pm
Thursday: 8:30am - 2:00pm
Friday: 8:30am - 3:30pm
