What are the responsibilities and job description for the Social Worker position at Champion Care?
St Ann Health and Rehab is hiring for a passionate Social Worker (LSW) to join our 50-Bed Skilled Nursing facility. As a Social Worker, you will work closely with our residents to ensure quality care is provided to improve the lives of our residents.
Duties and Responsibilities:
Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs
Identifying individual social and emotional needs
Developing and maintaining individualized care plans
Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
Work with the finance team to ensure resident is educated and the proper documents are in place for the resident during their stay
Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community
Qualifications/Requirements:
Degree in Social Worker
Licensed Social Worker (LSW) is preferred
At least one year experience in Social Work in Long Term Care or similar settings
Experience working with a diverse aging population
