What are the responsibilities and job description for the Telehealth Intake Assessor position at Champion Healthcare Services?
Mental Health Agency is looking for Licensed clinicians to conduct assessments. The Intake Assessor is responsible for conducting initial assessments on new clients for mental services. Assessments can be completed Telehealth due to COVID-19 precautions. This individual is responsible for coordinating and scheduling all intake appointments and obtain all necessary consent forms.
Prior assessment writing experience is a must. Must also have experience reviewing intake paperwork with families.
The following are responsibilities that the Assessor shall provide for the organization:
Responsible for conducting an initial intake on assigned clients
Responsible for determining client’s diagnosis based on diagnostic criteria from DSM-5.
Responsible for administrative duties related to intake/assessment activities.
Responsible for developing treatment plan in collaboration with the client and clinical team
Responsible for documenting progress notes
