This Practice RN position is responsible for participating in patient assessment and providing care, education, and instruction to patients using symptom-based telephone and in-person triage. This is a full time 40 hour per week position typically working during normal clinic hours (typically Monday through Friday from 8 a.m. to 5 p.m.). Some evenings and Saturdays may be required.

Requirements:
- Licensure with the State of Michigan as a Registered Nurse required.
- One year professional clinical nursing experience; family practice experience preferred or an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job listed above.
Responsibilities:
- Complete patient assessment, education and, as appropriate, patient scheduling telephonically and in person through walk-in appointments.
- Utilize knowledge in conjunction with approved protocols and standards in advising patients and parents.
- Conduct one-on-one or family patient education sessions.
- Ensure call-backs are made as appropriate.
- Assist in coordinating services for patients and families utilizing other health-related agencies and resources. Provide follow-up for patients on health care issues.
- Participate in interdisciplinary planning for meeting patient needs and health service objectives.
- Document nursing action in medical records utilizing defined standard.
- Assist in formulating and carrying out policies, procedures, rules, regulations, guidelines, and protocols.
- Participate in the quality assurance program.
- May be required to translate other languages if applicable.
- Understands and adheres to CSHS’s compliance standards as they appear in CSHS’s Corporate Compliance Policy, Code of Conduct and Conflict of Interest Policy.
- Keeps abreast of all pertinent federal, state and CSHS’s regulations, laws, and policies as they presently exist and as they change or are modified.
- Ensures that the staff are trained and evaluated on their knowledge of and adherence to compliance policies and procedures specific to their jobs.
