Since inception, CHI St. Alexius Health Turtle Lake has been dedicated to serving the residents of Turtle Lake, ND and the surrounding rural communities. CHI St. Alexius Health includes the Turtle Lake Hospital, Turtle Lake and Washburn Family Clinics. CHI St. Alexius Health Turtle Lake includes services of a 26-bed facility, a rural health clinic, and an emergency department that is available 24 hours a day. We offer many outpatient services, including physical therapy, radiology services, laboratory services, cardiac rehabilitation, and IV treatments.
Responsibilities
Job Summary / Purpose
To provide medically related social services to the residents and patients of CHI St. Alexius Health- Turtle Lake. Social services are provided to ensure that each resident attain and maintain their highest level of physical, mental and psycho-social functioning, while follow policy and procedures and the Mission and Philosophy of the facility.
Essential Key Job Responsibilities
- Organize and implement a social service program within the hospital acute care and swing bed units as it pertains to acute care patients, full time residents, and also respite and adult day care participants. Reviews information with appropriate staff to assess ability to handle the care and meet needs. Maintains waiting list. Responsible to inform Medical Provider and Nursing of intake information on potential residents and respite care or adult day care participants.
- Reviews all patient charts on admission or as soon as practical for appropriateness of admissions and the level of care to be provided. Charts on initial care plan and assess the discharge potential, rehab potential and discharge options.
- Resident Advocate: Explains all admission papers to residents and/or family. Inform resident and/or family of resident’s Bill of Rights and the North Dakota Long Term Care Ombudsmen Law on admission. Acknowledge of the receipt of this documented by a signature. Provides required information about facility policies, procedures, etc. to residents and families on admission and thereafter as required. Identifies and offer social and emotional support to residents and families. Handle grievances in appropriate manner. Promotes residents rights.
- Arranges for appointments/transportation: Assists with scheduling appointments and arranging for transportation for residents as needed.
- Reviews each inpatient admission and assess for need of pre-certified and continued stay requirements and documents results in discharge planning notes. Review care plans every 30 days for the first three months, quarterly thereafter. Notifies resident and family about conferences.
- Discharge planning: provides discharge planning for all patients admitted in acute and swing bed. Provides notice 48 hours for discharge or change in level of care per CMS guidelines. Completes notices, provides documentation, makes referrals and arranges transportation if needed.
- Meets quarterly with Social Worker and follows recommendation for patients.
- Provides in-services and education on intra-hospital and community groups including yearly in-service on Residents Rights for 100% of all hospital employees.
- Assures development of programs, policies, and procedures that respond to the changing time in healthcare: Policies and Procedures updated per policy.
- Perform other duties as assigned.
Qualifications
It is desirable that a qualified Social Worker Designee should have at least one year of experience in a health related facility or a social agency dealing with aging.
