The Children's Home is seeking a passionate Case Manager to become part of our new Tioga and Tompkins Counties Youth Assertive Community Treatment (ACT) program!
The ACT Program:
Utilizes an interdisciplinary team approach to serve children between the ages of 10-21 years old with mental health needs. The Youth ACT program is an alternative to out-of-home placement, is family driven, and serves the purpose of improving the functioning of children at home, in school and in the community!

What you will be doing in your role:
The Case Manager will be a part of an interdisciplinary team that provides support to youth and family in acquiring skills and abilities to manage their health, improve family relationships, and develop opportunities for educational, vocational, and prosocial activities and interactions.
- Work as part of an interdisciplinary team
- Engage youth and their families in their home/community
- Provide skill development
- Educational Support
- Vocational Support
- Support in accessing services such as housing, insurance, healthcare, etc.
- Linkages to prosocial activities
Your Education and Qualifications:
- Bachelor's Degree in Human Services required
- 2 years relevant experience
- Valid Driver's License and ability to maintain insurance throughout employment required
