- Patient Care Management
- Provides/coordinates health care to Fetal Heart Program (FHP) patients/families in the CHOP outpatient fetal heart clinic.
- Assess health status and identifies health care needs of pregnant women and their families.
- Develop/facilitate a comprehensive patient care management plan in collaboration with the medical team and other appropriate health care members.
- Communicates plan of care and proactively manage/resolve to meet quality/cost standards.
- Provides counseling and crisis intervention as necessary to patients and families.
- Fosters continuity of care within the health system (CHOP and HUP) and serve as a patient/family advocate.
- Provide/coordinate care and management to Program patients in outpatient and inpatient areas (which may include evening or weekend responsibilities).
- Provide instruction in disease process and medical plan to patient/family.
- Assess patient psychosocial and support needs, initiating appropriate referrals.
- Participate in appropriate rounds relevant to patient’s management i.e. FHP weekly patient review.
- Participate in the development of patient management plans to proactively manage and meet quality and cost standards.
- Helps to introduce families to the services of the Cardiac Center (i.e. coordinate tours of facilities, introduction to CT Surgery staff, CT Anesthesia staff, and Interventional and Electrophysiology Cardiology staff.
- Assist patients in the transition between prenatal care/evaluation and post-natal care/evaluation.
- Track physician/patient referral patterns.
- Provides/coordinates health care to Fetal Heart Program (FHP) patients/families in the CHOP outpatient fetal heart clinic.
- Education: Contribute to the ongoing educational and professional development of staff, students and residents in the area of prenatal cardiac diagnosis and treatment.
- Participate in educational activities which promote quality patient care.
- Precept nursing students.
- Attend appropriate FHP patient management conferences.
- Present at lectures, seminars and/or conferences.
- Participate in the development of patient educational materials.
- Participate in regional CME conferences
- Consultation: Function as a consultant to nursing staff, physicians, and others in the area of fetal cardiovascular care as well as support and ancillary staff
- Consult with health care providers within the hospital and community regarding practice standards

- Research
- Contribute to expansion of the body of knowledge in the fetal cardiovascular area.
- Evaluate and disseminate research findings.
- Utilize research findings in own practice and encourage staff to do the same.
- Participate in clinical research.
- Contribute to new knowledge and the literature.
- Professionalism: Maintain current knowledge and competency in the area of fetal, neonatal and pediatric cardiology and cardiac surgery in professional practice.
- Meet Department of Nursing and other regulatory agency requirements including licensure, quality improvement, infection and environmental control, and safety.
- Participate in writing of standardized procedures, protocols, patient care guides and annual revisions.
- Participate in professional organizations
- Contribute to the building and maintenance of optimal multidisciplinary team function and spirit within a group of highly dedicated professionals including nurses, physicians, sonographers and social workers, all focused on optimizing outcomes for the fetus with cardiovascular disease and its family
- Three (3) years of CICU and/or comprehensive pediatric cardiology or pediatric cardiac operating room or pediatric cardiac catheterization lab experience.
- Previous experience in complex clinical settings that required coordination of multiple services in a multidisciplinary fashion
- Critical analysis and problem-solving skills
- Ability to work independently
- Ability to work collaboratively through consultation with other health care team providers
- Ability to communicate verbally and in written English
