What are the responsibilities and job description for the Wraparound Facilitator position at Children's Legacy Center?

The Children’s Legacy Center (CLC) is seeking a Wraparound Facilitator to join our team. The role of the Facilitator is responsible for the initial engagement of the youth and caregivers, initial and ongoing assessments of needs and strengths, and the initial formation of each family’s personal Wraparound team. The Facilitator is the point of contact for all service providers and professionals involved in the Wraparound process, and will liaise with these professionals, family members, and the rest of the team to develop the Wraparound service plan.


Wraparound services are holistic family centered team-based interventions focused on providing evidenced based mental health, behavioral health, and mentoring services to a wide variety of youth and caregivers. As part of the Wraparound team, the Wraparound Facilitator will work closely with clinical professionals, county child welfare, juvenile probation, resource families, biological parents, adoptive parents, and various other caregivers to support family growth and placement preservation. The Children’s Legacy Center specializes in serving a demographic of youth who have been or are at risk of commercial sexual exploitation of children (CSEC) and as such, this position will require specialized training in meeting the unique needs of CSEC youth. This position reports to the Wraparound Program Manager.

Requirements -

  • Bachelor’s Degree and six months of experience in working with children who have serious emotional or behavioral needs, or children who have special needs, including, but not limited to, special medical needs; OR an Associate degree and one year of experience in working with children who have serious emotional or behavioral needs, or children who have special needs, including, but not limited to, intensive medical needs
  • Able to collaborate in team settings
  • Ability to connect with families from diverse backgrounds and cultures
  • Knowledge of culturally competent practices an ability to demonstrate cultural humility
  • Strong interpersonal skills

Preferred Qualifications -

  • Master’s Degree
  • Bilingual
  • Demonstrate knowledge and understanding of systems of care for diverse youth and families including, child welfare and/or juvenile justice
  • Excellent verbal and written communication skills

Role and Responsibilities -

  • Engages youth and caregiver in the Wraparound Process through active and trauma informed engagement to understand current circumstances, family strengths, and needs
  • Orients the youth and caregiver to the Wraparound process
  • Assists the youth and caregiver in identifying potential members of the Wraparound team
  • Gathers information from multiple sources regarding the strengths and needs of the youth and caregiver and uses that information to ensure accurate information is informing the Wraparound process and plan
  • Completes evidenced based assessments including the child and adolescent needs and strengths assessment (CANS) and the Commercial Sexual Exploitation Identification Tool (CSE-IT)
  • Schedules and facilitates Child and Family Team Meetings
  • Develops the initial Wraparound Service plan ensuring strategies align with cultural values, interests, and preferences
  • Provides coordination of the plan ensuring team members are aware of follow-up actions
  • Leads the team in reviewing plan and evaluation of the strategies utilized
  • Presents information at a weekly team meeting
  • Conducts family finding to locate and engage natural supports
  • Assists in after-hours crisis response as needed

**The Children’s Legacy Center began in 2016. Our passion is to aid children and youth who have been abused and exploited. We currently achieve this goal by offering forensic interviewing services as a part of our Children’s Advocacy Center. These interviews reduce trauma and help our partner agencies expedite justice for children, youth, and their families. We are now able to build upon this tested and stable foundation and are building out our regional pilot model for exploited children and youth. This model is a diverse, holistic, and far-reaching collection of programs and services that include a fully-staffed Behavioral Health Clinic (Center for Resilience), Advocacy and Wraparound Services, and Foster Family Agency, will multiply our impact in the community and beyond. Although each of our team obtains a specific job description, we work collaboratively to develop and expand our organization.
