What are the responsibilities and job description for the Care Coordinator position at CHR?
- Coordinate Care for people with substance use disorders including engagement in treatment, coordinating care internal and external providers, completing toxicology screens, referring for recovery services and medical providers, education about medication assisted treatment, education about naloxone, coordinating with referral sources.
DUTIES & RESPONSIBILITIES:
- Engage people with substance use disorders in treatment, particularly medication assisted treatment for people with Opioid Use Disorders and Alcohol Dependence.
- Orient new clients to treatment services, including providing education about MAT, overdose prevention, counseling.
- Monitor substance use by developing relationships with people, performing toxicology screens, accessing laboratory results, gathering information from other treatment providers.
- Engage family and natural supports in treatment, including making referrals to self-help communities.
- Ensure that people have access to recovery resources, including transportation, benefits, housing, employment, social supports to enhance recovery.
- Provide education about MAT, relapse prevention, overdose prevention, addiction.
- Coordinate care with external and internal providers, including medical and dental care.
- Perform all documentation according to agency standards.
- Other duties as assigned.
QUALIFICATIONS:
Education: Bachelor’s Degree in Behavioral health or 3 years relevant experience required. .
Experience: Excellent communication skills. Excellent computer skills. Experience working with individuals in substance abuse treatment who may also have co-occurring mental health disorders preferred.. Minimum 2 year experience in the field of care coordination preferred. Licensure/Certification/Registration: Valid driver’s license required.
